摘要
目的:基于中性粒细胞/淋巴细胞比值(NLR)观察庚子三号方治疗重型新型冠状病毒肺炎(内闭外脱证)患者的临床疗效。方法:采用回顾性研究的方法,选取2020年2月1日至2020年5月31日在湖北省中西医结合医院江北分院救治定点医院收治的新冠病毒肺炎重型患者50例作为研究对象,根据临床干预方式不同分为两组各25例。西药组予抗细菌、抗病毒、营养支持、免疫支持治疗,中药组加用庚子三号方治疗。比较两组中医证候积分、NLR、核酸转阴时间、CT好转率、总病程的差异。结果:治疗第7天,中药组中医证候积分优于西药组;中药组中性粒细胞计数、淋巴细胞计数、NLR均恢复正常范围且优于西药组;出院时中药组核酸转阴时间、CT好转率、总病程均优于西药组(P﹤0.05)。结论:庚子三号方参与治疗新型冠状病毒肺炎重型患者较单纯西医治疗疗效更好,能明显改善患者呼吸道症状,缩短疾病病程,减低副作用;NLR可作为敏感有效的辅助指标评估疗效。
Objective:To observe the clinical efficacy of Gengzi No.3 recipe on inner blocking causing collapse syndrome caused by severe COVID-19,based on neutrophils to lymphocyte ratio(NLR).Methods:50 cases of patients with severe COVID-19 were selected as researched objects by the method of retrospective study,and divided into two groups according to different clinical intervention methods,25 cases in each group.Western medicine group was treated with antibacterial and antiviral therapy,nutritional support and immune support,while traditional Chinese medicine(TCM)group was treated plus with Gengzi No.3 recipe on the basis of western medicine group.The differences of following indexes were compared between two groups,such as TCM syndrome score,NLR,negative conversion time of two nucleic acid reexaminations,CT improvement rate and total course of disease.Results:On the 7th day of treatment,TCM syndrome score of TCM group was better than that of western medicine group;neutrophils count,lymphocyte count and NLR of TCM group all got to the normal range,and better than those of western medicine group;At discharge,negative conversion time of nucleic acid,CT improvement rate and total course of disease in TCM group were better than those in western medicine group(P﹤0.05).Conclusion:The clinical efficacy of Gengzi No.3 recipe on severe COVID-19 is better than that of simple western medicine,which can significantly improve the respiratory symptoms of patients,shorten the course of disease,reduce the side-effects.NLR can be used as a sensitive and effective auxiliary index to evaluate the curative effect.
