摘要
目的:探讨透刺法“百会”透“太阳”穴治疗高血压性脑出血微创清除术后患者的临床疗效。方法:选择黑龙江中医药大学附属第一医院急诊科收治符合纳入标准的高血压性脑出血患者128例,所有入选患者均采用微创清除术进行救治,术后采用随机数字表将患者分为治疗组和对照组。术后两组患者均采用常规的术后护理方案,包括降颅内压、控制血压、改善微循环、营养神经等,对照组给予尼莫地平和麝香注射液。治疗组在对照组的基础上应用透刺法“百会”透“太阳”穴进行治疗。两组均治疗28 d。观察两组治疗前后,美国国立卫生研究院卒中量表(NIHSS)评分和中医证候积分情况以及血清HSP和TNF-α的变化情况。结果:治疗前两组患者NIHSS评分和中医证候积分没有统计学意义(P>0.05),治疗后两组评分均降低,且治疗组优于对照组(P<0.05)。治疗前HSP、TNF-α水平比较,没有统计学意义(P>0.05),治疗后,HSP水平均升高,TNF-α均下降,且治疗组优于对照组(P<0.05)。结论:“百会”透“太阳”穴的头刺法可以显著缓解患者的临床症状,其机制可能与升高HSP水平、降低TNF-α水平有关。
Objective:To explore the effect of Baihui(DU 20)through Taiyang(EX-HN5)acupuncture in the treatment of hypertensive intracerebral hemorrhage after minimally invasive surgery.Methods:128 patients with hypertensive intracerebral hemorrhage were included in the study.All patients were treated with minimally invasive surgery.Afterwards,the patients were randomly divided into the treatment group and the control group.The patients in the two groups were treated with routine postoperative and nursing programs,including reducing intracranial pressure,controlling blood pressure,improving microcirculation and nourishing nerves,the control group was also treated with Nimodipine and Shexiang Injection;on which basis,the treatment group was also treated with DU 20 through EX-HN5 acupuncture.Both groups were treated for 28 days.The scores of NIHSS and TCM syndrome,as well as the serum levels of HSP and TNF-αwere observed before and after the treatment in the two groups.Results:There were no statistical differences in the scores of NIHSS and TCM syndrome between the two groups before the treatment(P>0.05);the scores were decreased after the treatment in the two groups(P<0.05),and the decreases in the treatment group were more significant(P<0.05).There were no statistical differences in the levels of HSP and TNF-αbetween the two groups before the treatment(P>0.05);the level of HSP was increased and the level of TNF-αwas decreased after the treatment in the two groups(P<0.05),and the improvements in the treatment group were more significant(P<0.05).Conclusion:Baihui(DU 20)through Taiyang(EX-HN5)scalp acupuncture can significantly relieve the clinical symptoms of the patients,and its mechanism may be related to the increase of HSP and the decrease of TNF-α.
