摘要
【目的】观察硫辛酸联合阿司匹林治疗2型糖尿病下肢血管病变的疗效。【方法】选取我院于2018年8月至2020年10月收治的2型糖尿病下肢血管病变患者104例,参照其治疗方案进行均分组,对照组52例给予阿司匹林治疗,观察组52例给予硫辛酸联合阿司匹林治疗。比较两组总有效率和不良反应率,记录两组血管内皮生长因子(VEGF)、内皮素(ET)、高敏-C反应蛋白(hs-CRP)等血清因子的表达,观察两组踝肱指数(ABI)、心脏-脚踝指数(CAVI)及神经病变主觉症状问卷(TSS)评分的变化。【结果】观察组总有效率高于对照组,且有统计学意义(P<0.05)。治疗后两组VEGF、ET、hs-CRP等血清因子均大幅下降(P<0.05),ABI、CAVI等均大幅提升(P<0.05),且观察组治疗后VEGF、ET、hs-CRP较对照组更低(P<0.05),ABI、CAVI较对照组更高(P<0.05)。治疗后两组疼痛、灼热感、感觉异常及麻木等评分均大幅下降(P<0.05),且观察组治疗后各项TSS评分较对照组更低(P<0.05)。两组不良反应主要为胃部不适、恶心呕吐,两组比较无统计学意义(P>0.05)。【结论】硫辛酸联合阿司匹林治疗2型糖尿病下肢血管病变可降低VEGF、ET、hs-CRP等血清因子的表达,改善足部微循环,提高疗效,且安全性高。
【Objective】To observe the curative effect of lipoic acid combined with aspirin on type 2 diabetic patients with lower extremity vascular disease.【Methods】104 patients with type 2 diabetic lower extremity vascular disease admitted to our hospital from August 2018 to October 2020 were selected and divided into two groups according to the treatment plan.52 cases in the control group were treated with aspirin,and 52 cases in the observation group were treated with lipoic acid combined with aspirin.The total effective rate and adverse reaction rate of the two groups were compared.The expression of vascular endothelial growth factor(VEGF),endothelin(ET),high sensitivity C-reactive protein(hs-CRP)and other serum factors in the two groups were recorded.The changes of ankle brachial index(ABI),heart ankle index(CAVI)and neuropathy subjective symptom questionnaire(TSS)scores in the two groups were observed.【Results】The total effective rate of the observation group was higher than that of the control group(P<0.05).After treatment,VEGF,ET,hs-CRP and other serum factors in the two groups were significantly decreased(P<0.05),ABI,CAVI were significantly increased(P<0.05),and VEGF,ET,hs-CRP in the observation group were lower than those in the control group(P<0.05),ABI,CAVI were higher than those in the control group(P<0.05).After treatment,the scores of pain,burning sensation,paresthesia and numbness in the two groups decreased significantly(P<0.05),and the TSS scores in the observation group were lower than those in the control group(P<0.05).The main adverse reactions of the two groups were stomach discomfort,nausea and vomiting,and there was no statistical significance between the two groups(P>0.05).【Conclusion】Lipoic acid combined with aspirin can reduce the expression of serum factors such as VEGF,ET and hs-CRP in patients with type 2 diabetic lower extremity vascular disease,improve the microcirculation of foot,improve the curative effect,and have high safety.
