摘要
目的探讨小儿肠套叠高频彩色多普勒超声诊断价值.方法本文选择我院2019年10月~2020年10月收治的168例患儿,所有患儿均给予腹部X线检查及高频彩色多普勒超声检查.观察描述不同探头频率下彩色多普勒超声图像结果及灌肠复位结果,对比不同检测方法的诊断准确率.结果常规探头显示低回声包块,回声不均匀,近端肠管不同程度扩张状态.高频彩色多普勒超声图像显示,横切面呈"同心圆"、"靶环征",边缘轮廓不规则;纵切面呈"假肾征",周边低回声带,套入部肠管及肠系膜高低混合回声,中心部分肠内容物可见气体强回声;套入肠管周围可见肠系膜淋巴结,"椭圆形",低回声.所有168例患儿均在超声引导下生理盐水灌肠复位,165例(98.21%)复位成功,3例(1.79%)复位失败转手术治疗.腹部X线诊断肠套叠137例,诊断准确率为81.55%;高频彩色多普勒超声诊断肠套叠166例,诊断准确率为98.81%;两组诊断准确率比较,差异有统计学差异(χ^(2)=28.260,P=0.000).结论高频彩色多普勒超声诊断小儿肠套叠准确率高,可清晰显示患儿超声图像特征,为患儿临床治疗提供可靠依据,操作简单,安全性高,值得推广.
Objective This study aims to investigate the value of high frequency color Doppler ultrasound in the diagnosis of intususception in ehildren.Methods 168 eases of children in our hospital from October 2019 to Oetober 2020 were selected.All children were given abdominal X-ray examination and high frequency color Doppler ultrasound examination.It observed and deseribed the results of color Doppler ultrasound images and enema reduction resuls under different probe frequencies,and compared the dliagnostic aceuracy of different deteetion methods.Results The conventional probe showed hy poechoic mass,inhomoge neous echo and different degree of proximal intestinal dilatation.High frequency color Doppler ultrasound images showed thal the transverse section showed"eoncentrie cirele"and"larget ring sign"with irregular edge contour.In addition,the longiudinal seetion showed"pseudo kidney sign"with peripheral low erho,high and low mixed echo of intususcept ion intestine and mesentery,with strong gas echo in the central part of intestine.Meanwhile,the mesenterie ly mph nodes around intususception intestine were"oval"and low echo.All 168 cases were treated with normal saline enema under ultrasound guidance.Of them.165 cases(98.21%)were succssfully treated,and 3 cases(1.79%)were converted to surgery.Abdominal X-ray diagnosis of intussusception in 137 eases,the diagnostie aeeuracy was 81.55%;which was statistically lower that of the diagnostie accuraey of 98.81%for high frequency color Doppler ultrasound diagnosis of intussusception in 166 cases(χ^(2)=28.260,P-=0.000).Conc lus ion The high frequency color Doppler ultrasound diagnosis of intussusception in children with high accuracy.It elearly shows the characteristics of ulrasound images in children.pro ides reliable hasis for clinical treatment of children.
