不同剂量熊去氧胆酸治疗早产儿黄疸的临床观察

摘要 目的探讨熊去氧胆酸治疗早产儿黄疸的最佳应用剂量。方法选择2014年8月至2018年12月在国药东风总医院诊治的早产儿黄疸96例,按照入院顺序,根据随机数字表法分为A、B、C 3组各32例,A组、B组与C组分别口服熊去氧胆酸10 mg/kg·d、20 mg/kg·d、30 mg/kg·d,1次/d,治疗4周,观察患儿临床疗效、不良反应、丙氨酸氨基转移酶(ALT)、天门冬氨酸氨基转移酶(AST)、总胆红素(TBIL)与直接胆红素(DBIL)及白细胞介素-6(IL-6)与肿瘤坏死因子(TNF-α)。结果3组患儿的治疗总有效率分别为93.8%、96.9%和93.8%;治疗期间只有7.29%的患儿出现轻微皮疹、腹泻、便秘等不良反应;治疗后的ALT、AST、TBIL、DBIL、IL-6、TNF-α水平都显著低于治疗前,差异有统计学意义(P<0.05),但3组间比较差异无统计学意义(P>0.05)。结论熊去氧胆酸治疗早产儿黄疸时,可有效发挥护肝作用。在推荐使用剂量范围以内(10~30 mg/kg·d),临床效果显著,无显著毒副作用。 Objective To investigate and analysis the optimal dose of ursodeoxycholic acid in the treatment of jaundice in premature infants.Methods Ninety-six cases of jaundice in premature infants treated in Dongfeng General Hospital of Chinese medicine from August 2014 to December 2018 were selected and randomly divided into three groups according to the order of admission,32 cases in each group.Group A,group B and group C were treated with ursodeoxycholic acid 10 mg/kg·d,20 mg/kg·d and 30 mg/kg·d,once a day for 4 weeks.The clinical efficacy,adverse reactions,alanine aminotransferase(ALT),alanine aminotransferase(ALT),Aspartate aminotransferase(AST),total bilirubin(TBIL)and direct bilirubin(DBIL),interleukin-6(IL-6)and tumor necrosis factor(TNF)were measured-α were observed.Results The total effective rates of 3 group were 93.8%,96.9%and 93.8%respectively;During the treatment,only 7.29%of the children had mild rash,diarrhea,constipation and other adverse reactions;The levels of ALT,AST,TBIL,DBIL,IL-6 and TNF-αafter treatment were significantly higher than those before treatment,the difference was statistically significant(P<0.05),but there was no significant difference among the three groups(P>0.05).Conclusion Ursodeoxycholic acid can effectively protect the liver in the treatment of jaundice in premature infants.In the recommended dosage range(10~30mg/kg.D),the clinical effect is significant,without significant side effects.
