摘要
目的观察缺血性脑卒中患者体内血浆脂蛋白相关磷脂酶A2(Lp-PLA2)的变化。方法随机选取2019年1月—2020年1月在该院明确诊断为急性缺血性脑卒中的患者共计152例作为研究组,采用电脑随机方法在同期于该院体检的健康人群中随机选取150名作为对照组,分别比较两组体质量指数(BMI)、吸烟史、饮酒史及Lp-PLA2数值。对两组患者的Lp-PLA2进行Logistic回归和ROC分析,了解其相关性。对研究组的预后结果再次分成两个亚组,比较其大面积梗死、Lp-PLA2数值差异。结果研究组和对照组BMI、吸烟史、饮酒史、Lp-PLA2对比,差异有统计学意义(P<0.05);对缺血性脑卒中的相关风险因素进行多因素Logistic回归分析显示,Lp-PLA2的OR值为1.059(95%CI1.043~1.074)。研究组和对照组ROC分析显示,AUC值为0.917(95%CI为0.880~0.946),Youden指数0.705,截断值165.94 ng/mL。研究组中两个不同亚组显示,大面积梗死、Lp-PLA2与患者预后密切相关(P<0.05)。结论血清Lp-PLA2水平与急性缺血性脑卒中密切相关,动态监测其指标变化不仅能指导急性缺血性脑卒中的诊治,也有助于其预后判断。
Objective To observe the changes of plasma lipoprotein-related phospholipase A2(Lp-PLA2)in patients with ischemic stroke.Methods A total of 152 patients with a clear diagnosis of acute ischemic stroke in the hospital from January 2019 to January 2020 were randomly selected as the study group.A computer randomized method was used to randomly select 150 from the healthy population who had a physical examination in the hospital during the same period.Names served as a control group to compare the BMI,smoking history,drinking history,and Lp-PLA2 values between the two groups.Logistic regression and ROC analysis were performed on Lp-PLA2 of the two groups of patients to understand its correlation.The prognostic results of the study group were divided into two subgroups again,and the differences in infarct size and Lp-PLA2 values were compared.Results There was a statistically significant difference between study group and the control group of BMI,smoking history,drinking history,and Lp-PLA2(P<0.05).Multivariate Logistic regression analysed on the related risk factors of ischemic stroke showed that the OR value of Lp-PLA2 was 1.059(95%CI 1.043-1.074).ROC analysis of the study group and control group showed that the AUC value was 0.917(95%CI 0.880-0.946),the Youden index was 0.705,and the cutoff value was 165.94 ng/mL.Two different subgroups in the study group showed that the infarct size and Lp-PLA2 were closely related to the prognosis of patients(P<0.05).Conclusion The level of serum Lp-PLA2 is closely related to acute ischemic stroke.Dynamic monitoring of its index changes can not only guide the diagnosis and treatment of acute ischemic stroke,but also help its prognosis.
