摘要
目的探讨亮丙瑞林与孕三烯酮片对子宫内膜异位症(EMT)术后患者生殖激素水平及短期复发的影响。方法便利选择2019年1月—2020年1月在该院行子宫内膜异位症手术治疗患者97例作为研究对象,采用随机数表法的原则将其分为A组(n=48)及B组(n=49)。A组给予孕三烯酮片治疗,B组给予亮丙瑞林治疗。比较分析两组临床疗效,治疗前、治疗3个月后的生殖激素水平及短期复发情况。结果B组治疗总有效率(97.96%)高于对照组(83.33%),差异有统计学意义(χ^(2)=4.547,P=0.013);治疗后,相比A组促卵泡生成激素(5.42±1.29)mmol/mL、促黄体生成激素(5.79±1.24)mmol/mL、雌二醇(86.49±10.41)pg/mL,B组促卵泡生成激素(3.79±1.16)mmol/mL、促黄体生成激素(4.05±1.01)mmol/mL、雌二醇(75.42±9.45)pg/mL水平均低,差异有统计学意义(t=6.547、7.585、5.486,P<0.001);B组不良反应(4.08%)略低于A组(10.42%),但差异无统计学意义(χ^(2)=0.661,P=0.228);B组复发率(4.08%)明显低于A组(18.75%),差异有统计学意义(χ^(2)=5.189,P=0.023)。结论亮丙瑞林较孕三烯酮片治疗EMT术后患者疗效更佳,有利于抑制生殖激素水平,降低术后短期复发率,且未见明显不良反应增加,安全可行。
Objective To investigate the effects of leuprolide and gestrinone tablets on reproductive hormone levels and short-term recurrence in patients with endometriosis after surgery.Methods Ninety-seven patients who underwent endometriosis surgery in the hospital from January 2019 to January 2020 were conveniently selected as the research objects,and they were divided into group A(n=48)and group B(n=49)using the principle of random number table method.Group A was treated with gestrinone tablets,and group B was treated with leuprolide.The clinical effects,the reproductive hormone levels before and after 3 months of treatment,and short-term recurrence of the two groups were compared and analyzed.Results The total effective rate of treatment in group B(97.96%)was higher than that of the control group(83.33%),and the difference was statistically significant(χ^(2)=4.547,P=0.013);after treatment,compared with group A follicle-stimulating hormone(5.42±1.29)mmol/mL,luteinizing hormone(5.79±1.24)mmol/mL,estradiol(86.49±10.41)pg/mL,group B follicle-stimulating hormone(3.79±1.16)mmol/mL,luteinizing hormone(4.05±1.01)mmol/mL,and estradiol(75.42±9.45)pg/mL all lower,and the difference was statistically significant(t=6.547,7.585,5.486,P<0.001);adverse reactions in group B(4.08%)were slightly lower than those in group A(10.42%),but the difference was not statistically significant(χ^(2)=0.661,P=0.228);the recurrence rate of group B(4.08%)was significantly lower than group A(18.75%),the difference was statistically significant(χ^(2)=5.189,P=0.023).Conclusion Leuprolide is more effective than gestrinone in the treatment of patients with EMT after surgery.It is beneficial to inhibit the level of reproductive hormones and reduce the short-term recurrence rate after surgery.There is no significant increase in adverse reactions.It is safe and feasible.
