摘要
目的:观察祛毒生肌膏纱条联合TDP照射治疗对促进复杂性肛瘘术后创面愈合及对血清TNF-α、IL-6和IL-10的影响。方法:选取在我院因肛瘘入院,行肛周MRI检查,结合临床表现诊断为复杂性肛瘘,且行相关手术治疗的患者作为研究对象,根据纳入、排除和剔除三重标准选取符合条件的84例患者并进行分组,分组过程遵循随机的原则,依据数字表达法平均分为观察组和两组对照组。其中观察组采用祛毒生肌膏纱条填塞+TDP照射治疗,对照组1采用单纯祛毒生肌膏纱条填塞,对照组2采用凡士林纱布填塞,观察三组患者术后治疗前后不同时间段疼痛VAS评分,每周血清TNF-α、IL-6促炎因子和IL-10抗炎因子含量变化,各类症状(包括创面分泌物、肉芽形态、红肿程度)评分,创面缩小率,腐肉脱落及创面愈合时间和临床疗效等,并如实记录相关数据。结果:治疗后观察组各类症状评分、血清TNF-α和IL-6含量变化、腐肉脱落及创面愈合时间均低于对照组,且三组患者以上指标均低于本组治疗前(P<0.05);治疗后观察组血清IL-10含量、创面缩小率、临床疗效均高于对照组(P<0.05)。结论:祛毒生肌膏纱条联合TDP照射治疗能改善复杂性肛瘘术后创面的各项症状表现,调节炎症因子含量的变化,保护创面以促进新陈代谢,达到去腐生新的临床目的,因此本方法值得被推广。
Objective:To observe the effect of Qudu Shengji Plaster and TDP irradiation on wound healing and serum TNF-α,IL-6 and IL-10 after complex anal fistula operation.Methods:select in November 2018~November 2019 in our hospital because of anal fistula,anus week MRI,combined with the clinical diagnosis of complex anal fistula,line and related surgical treatment of patients as the research object,according to include and exclude and eliminate the triple standard to select qualified to group and 84 patients,follow the principle of random grouping process,on the basis of digital expression divided into observation group and two control group on average.The observation group was treated with Qudu Shengji Plaster combined with TDP irradiation,Control group 1 was filled with simple Qudu Shengji Plaster,The control group 2 was filled with vaseline gauze,observe the different period of three groups of patients before and after the treatment of postoperative pain VAS score,serum TNF-αweekly,IL-6 proinflammatory factor and IL-10 anti-inflammatory factor content change,all kinds of symptoms,including wound secretion,granulation,swelling degree,score,shrink rate,rotten meat falls off,and the healing of the wound time,clinical curative effect,etal.,and faithfully record the related data.Results:After treatment,symptom scores,changes in serum TNF-αand IL-6 content,decaying flesh and wound healing time in the observation group were lower than those in the control group,and above indexes in the three groups were lower than those before treatment(P<0.05).After treatment,the serum IL-10 content,wound reduction rate and clinical efficacy in the observation group were all higher than those in the control group(P<0.05),and the differences were statistically significant.Conclusion:Qudu Shengji Plaster combined with TDP irradiation therapy can improve various symptoms of wound surface after complicated anal fistula surgery,regulate the changes of inflammatory factors,protect the wound surface to promote metabolism,and achieve the new clinical purpose of saprogenesis.Therefore,this method is worthy of promotion.
