摘要
目的探讨全踝关节置换术治疗终末期踝关节炎的短期疗效。方法回顾分析广州市正骨医院2016年10月—2020年10月采用INBONEⅡ全踝关节置换术患者10例,其中原发性踝关节炎6例,创伤性踝关节炎4例。术中联合距下关节融合1例,踝关节外侧韧带修复3例。采用美国足踝外科协会(American Orthopaedic Foot and Ankle Society,AOFAS)踝-后足评分、视觉模拟评分(visual analogue scale,VAS)及关节活动度(range of motion,ROM)进行疗效评定。结果术后10例患者全部得到随访,随访时间为11~16个月,平均(13.3±0.5)个月。术中出现内踝骨折1例,伤口延迟愈合1例,经换药后愈合,无伤口感染及坏死。术后随访无假体松动或下沉。患者末次随访AOFAS评分为(84.10±2.33)分,与术前(40.10±5.12)分相比,差异有统计学意义(P﹤0.01)。末次随访VAS评分为(1.23±0.60)分,与术前(7.13±0.56)分相比,差异有统计学意义(P﹤0.01)。末次随访踝关节ROM为(40.1°±8.7°),与术前(30.2°±5.5°)相比,差异有统计学意义(P﹤0.01)。结论INBONEⅡ全踝关节置换术治疗终末期踝关节炎具有缓解疼痛及改善踝关节功能的优点,且短期随访疗效满意,长期疗效有待进一步研究。
Objective To explore the short term effect of the total ankle arthroplasty in the treatment for end stage ankle arthritis.Methods The retrospective study contains 10 patients who were treated by INBONEⅡtotal ankle arthroplasty from October 2016 to October 2020 in Guangzhou Orthopedic Hospital,there were 6 cases of primary arthritis and 4 cases of traumatic arthritis.1 patient was in combined with a subtalar joint fusion and 3 patients were in combined with lateral ligament repair of the ankle.The evaluation for curative effect were AOFAS ankle hindfoot scale and VAS pain score as well as ROM of ankle.Results All the patients underwent the follow-up,The follow-up ranged from 11 to 16 months,average of(13.3±0.5)months.There was 1 case of medial malleolus fracture intraoperation,1 case of delayed wound healing who was treated by change dressing and gained healing,there was no wound infection and necrosis.No prosthesis loosening or subsidence was found at last follow-up.According to AOFAS ankle hindfoot scale,the score was(84.10±2.33)points at final follow-up,compared with preoperation score(40.10±5.12)points,the significance difference was made(P<0.01).The VAS score was(1.23±0.60)points at final follow-up,compared with preoperation score(7.13±0.56)points,the significance difference was made(P<0.01).The degree of ROM of ankle was(40.1°±8.7°)at final follow-up,compared with preoperation degree(30.2°±5.5°),the significance difference was made(P<0.01).Conclusion The INBONEⅡtotal ankle arthroplasty for ankle end stage arthritis had the advantage of pain relief and improvement of ankle motion,thus satisfaction of the shor term effect was observed.But there is a need for further analysis of its long term efficacy.
