摘要
目的:探究循环血miR类指标在脓毒症中的变化及其与严重程度的关系,以为脓毒症的临床诊治提供参考依据。方法:选取2019年6月-2021年2月本院收治的90例脓毒症患者为观察组,同时期的90名健康者为对照组。比较对照组和观察组的循环血miR类指标(miR-155-5p、miR-326、miR-223、miR-146a及miR-107)表达量,同时比较观察组中不同病因(肺部感染、腹腔感染及其他病因)及严重程度(脓毒症、严重脓毒症及脓毒性休克)者的上述循环血miR类指标表达量,采用Spearman秩相关分析上述循环血miR类指标与脓毒症严重程度的关系。结果:观察组的循环血miR-155-5p、miR-326、miR-223、miR-146a及miR-107表达量均显著高于对照组,差异均有统计学意义(P<0.05)。观察组中不同病因者的循环血miR-155-5p、miR-326、miR-223、miR-146a及miR-107表达量比较,差异均无统计学意义(P>0.05)。观察组中不同严重程度者的循环血miR-155-5p、miR-326、miR-223、miR-146a及miR-107表达量比较,差异均有统计学意义(P<0.05);脓毒性休克者的上述循环血miR类指标表达量均显著高于脓毒症者及严重脓毒症者,严重脓毒症者的上述循环血miR类指标表达量均显著高于脓毒症者,差异均有统计学意义(P<0.05)。Spearman秩相关分析显示,循环血miR-155-5p、miR-326、miR-223、miR-146a及miR-107均与脓毒症严重程度呈正相关(P<0.05)。结论:循环血miR类指标在脓毒症中呈现异常表达的状态,且与脓毒症严重程度密切相关,在脓毒症患者中的检测价值较高。
Objective:To explore the changes of circulating blood miR indexes in sepsis and its relationship with severity,so as to provide reference for clinical diagnosis and treatment of sepsis.Method:A total of 90 patients with sepsis admitted to our hospital from June 2019 to February 2021 were selected as the observation group,and 90 healthy subjects during the same period were selected as the control group.The expression levels of circulating blood miR indexes(miR-155-5p,miR-326,miR-223,miR-146a and miR-107)were compared between the control group and the observation group.At the same time,the expression levels of above circulating blood miR indexes in the observation group were compared with different etiologies(pulmonary infection,abdominal infection and other etiologies)and severity(sepsis,severe sepsis and septic shock).Spearman rank correlation was used to analyze the relationship between the above circulating blood miR indexes and the severity of sepsis.Result:The expressions of miR-155-5p,miR-326,miR-223,miR-146a and miR-107 in the observation group were significantly higher than those in the control group,the differences were statistically significant(P<0.05).There were no significant differences in the expression of miR-155-5p,miR-326,miR-223,miR-146a and miR-107 in patients with different etiologies of the observation group(P>0.05).The expressions of miR-155-5p,miR-326,miR-223,miR-146a and miR-107 in patients with different severity in the observation group were compared,the differences were statistically significant(P<0.05).The expression of above circulating blood miR indexes in septic shock group were significantly higher than those in sepsis group and severe sepsis group,the expression levels of above circulating blood miR indexes in severe sepsis were significantly higher than those in sepsis,the differences were statistically significant(P<0.05).Spearman rank correlation analysis showed that miR-155-5p,miR-326,miR-223,miR-146a and miR-107 in circulating blood were positively correlated with the severity of sepsis(P<0.05).Conclusion:Circulating blood miR indexes in sepsis showed abnormal expression,and is closely related to the severity of sepsis,which has high detection value in sepsis patients.
