摘要
目的采用数据挖掘方法探索总结王彦刚教授治疗胃癌前病变的用药规律及经验。方法采集王彦刚教授2017年2月1日—2018年12月31日门诊病例数据,并建立医案数据库,应用古今医案云平台,对医案数据库进行中药统计、中药属性分析、互信息法分析、聚类分析。结果共收集门诊处方169份,中药166味,高频药物有:黄芩(121次)、黄连(118次)、清半夏(114次)等;药味以苦、辛频次最高,药性以温为主,药物归经以肺、胃、肝经为主;功效以清热解毒、清热燥湿为主,绞股蓝-鸡骨草,板蓝根-鸡骨草,石膏-瓦楞子等为常用组合,聚类分析分为5类药物。结论王彦刚教授从“浊毒”治疗胃癌前病变,以辛开苦降、寒温并施为用药原则,针对形成浊毒的“湿”“滞”“痰”“瘀”“痈”等方面各个击破,同时善用温性药,调肺胃以理气机为主,重视“开窍”为其用药特色。
Objective To explore and summarize Professor Wang Yangang′s experience in the treatment of precancerous lesions of chronic atrophic gastritis by data mining.Methods Collect the outpatient case data of Professor Wang Yangang from February 1,2017 to December 31,2018,establish the data base of medical case,and apply the data mining method of ancient and modern medical case cloud platform software integration to carry out medication frequency,features,mutual information analysis and cluster analysis on the medical case database.Results A total of 169 out-patient prescriptions and 166 traditional Chinese medicines were collected.The frequency of high-frequency drugs was 121times,118 times,114 times respectively,which were Huangqin(Radix Scutellariae),Huanglian(Rhizoma Coptidis),Qingbanxia(Rhizoma Pinelliae)etc.The main taste of medicine is bitter and pungent;the si qi of medicine is mainly warm;the main channel of medicine is lung,stomach and liver;the main effect is to clear away heat and detoxify,clear away heat and dry dampness,Jiaogulan(Fiveleaf Gynostemma Herb)-Jigucao(Herba Abri),Banlangen(Radix Isatidis)-Jigucao(Herba Abri),Shigao(Gypsum Fibrosum)-Walengzi(Ark Shell),etc are commonly used combinations.Cluster analysis is summarized as five groups of drugs.Conclusion Professor Wang Yangang treats gastric precancerous lesions from“turbid toxin”.The medication is based on the principles of pungent prescribing and bitterness,cold and temperature combined with application,aiming at the“dampness”“stagnation”“phlegm”“stasis”and“carbuncle”that form the turbid toxin.Each is broken,and at the same time,warm medicine is used.The Qi regulating machine focuses on regulating the lungs and stomach,and emphasizes“resuscitation”as its medication feature.
