摘要
目的:观察从热瘀相兼论治消渴合并胸痹的临床疗效。方法:78例临床明确诊断的消渴合并胸痹[2型糖尿病合并冠状动脉硬化性心脏病(简称冠心病)]患者,随机分为治疗组(40例)和对照组(38例),两组均给予西医基础治疗,对照组在基础治疗的基础上加服用复方丹参滴丸。治疗组在基础治疗的基础上加用清热活血方。两组治疗疗程为6周,评估治疗前后中医症状积分、糖脂代谢情况、炎性因子水平[白细胞介素(Interleukin,IL)-6、肿瘤坏死因子-α(Tumor Necrosis Factor-α,TNF-α)、C反应蛋白(C-reactive Protein,CRP)]及心电图及射血分数(Ejection Fraction,Ef)变化情况。结果:治疗6周后,两组总有效率比较,差异具有统计学意义(P<0.05)。治疗组和对照组空腹血糖(Glucose,GLU)、餐后2小时血糖(2 h Postprandial Blood Glucose,2 h PBG)、糖化血红蛋白(Hemoglobin,HbA1c)均较治疗前降低,差异明显(P<0.01)。其中治疗组在降低HbA1c上和餐后血糖水平上同对照组相比较,显示出了更好的疗效(P<0.05)。对照组ST-T改变的有效率显著低于治疗组(P<0.05)。在改善心脏功能提高射血分数方面,治疗组也显示出了较好的疗效(P<0.05)。治疗组的CRP、TNF-α、IL-6较对照组降低更为显著(P<0.01)。结论:清热活血方能够改善消渴合并胸痹临床综合疗效、调节糖脂代谢、改善心脏供血、减轻低度炎症反应,提示从热瘀相兼的角度论治消渴合并胸痹具有较好疗效。
Objective:To observe the clinical effect of combination of heat and blood stasis therapy on diabetes with coronary heart disease.Methods:78 patients were randomly divided into the treatment group(40 cases) and control group(38 cases).All groups were given basic western medicine,and the control group was given the Danshen dripping pills(复方丹参滴丸) more.The treatment group was treated with the Qingre Huoxue prescription(清热活血方) more.Results:After 6 weeks,the total efficacy in the treatment group was higher than that the control group(P<0.05).GLU,2 h PBG and HbAlc in two groups were all lower than before treatment(P<0.01).HbAlc and PBG in the treatment group were better than the control group(P<0.05).The effective rate of ST-T changes in the control group was significantly lower than the treatment group(P<0.05).Cardiac function and ejection fraction in the treatment group also showed improved(P<0.05).CRP,TNF-α,and IL-6 in the treatment group were decreased more significantly than the control group(P<0.01).Conclusion:The Qingre Huoxue prescription was effective on diabetes with chest discomfort,by regulating blood sugar metabolism,improving blood supply to the heart,and reducing low-grade inflammation.
