摘要
目的研究口服避孕药(Oral Contraceptives,OC)及基础黄体生成素(Luteinizing Hormone,LH)水平对多囊卵巢综合征(polycystic ovary syndrome,PCOS)患者体外授精/卵母细胞胞浆内单精子注射(In Vitro Fertilization/Intracytoplasmic Sperm Injection,IVF/ICSI)妊娠结局的影响。方法前瞻性纳入2017年10月~2018年10月于我中心行IVF/ICSI治疗的PCOS患者70例。根据是否服用OC分为OC组与对照组,比较两组获卵数、受精率、第3天胚胎数及囊胚形成率,新鲜移植后胚胎着床率、临床妊娠率、流产率及活产率。并根据基础LH水平将对照组分为高LH组(LH≥10 IU/L)与低LH组(LH<10 IU/L),比较两组新鲜移植周期的妊娠结局。结果OC组与对照组间获卵数、受精率、D3胚胎数、囊胚形成率、胚胎种植率、临床妊娠率、流产率及活产率均无显著性差异。高LH与低LH组获卵数、受精率、D3胚胎数、囊胚形成率、胚胎种植率、临床妊娠率、流产率及活产率均无显著性差异。结论PCOS患者行拮抗剂方案IVF/ICSI前口服OC并不改善妊娠结局,基础LH水平不影响PCOS患者IVF/ICSI妊娠结局。
Objective To investigate the effects of oral contraceptives(OC)and luteinizing hormone(LH)levels on the outcome of patients with polycystic ovary syndrome(PCOS)undergoing in vitro fertilization/intracytoplasmic sperm injection(IVF/ICSI).Methods A total of 70 PCOS patients who underwent IVF/ICSI in our center from October 2017 to October 2018 were prospectively included.The number of retrievedoocytes,fertilization rate,number of embryos on day 3,blastocyst formation rate,implantation rate,clinical pregnancy rate,abortion rate and live birth rate were compared between the two groups.According to the basic LH level,the control group was divided into high LH group(LH≥10 IU/L)and low LH group(LH<10 IU/L).The pregnancy outcomes of the two groups were compared.Results There were no significant differences in the number of retrieved oocytes,fertilization rate,D3 embryo number,blastocyst formation rate,embryo implantation rate,clinical pregnancy rate,abortion rate and live birth rate between OC group and control group(0%vs.29.167%,P=0.07).There were no significant differences in the number of retrieved oocytes,fertilization rate,D3 embryo number,blastocyst formation rate,embryo implantation rate,clinical pregnancy rate,abortion rate and live birth rate between high LH and low LH groups(50%vs.22.22%,P=0.307).Conclusion Administration of OC before IVF/ICSI does not improve pregnancy outcome in PCOS patients.Basal LH level does not affect IVF/ICSI pregnancy outcome in PCOS patients.
