摘要
目的:研究咳嗽变异性哮喘(CVA)患者血清中sPLA2-X含量与CysLT、iNOS、IL-33及FeNO的关系,进一步研究sPLA2-X对CVA的影响。方法:选择贵州省人民医院呼吸与危重症医学科2018年6月至2019年11月门诊经确诊CVA的患者为研究对象及同期在我院健康体检中心体检的健康志愿者为对照组,分为CVA治疗前组(41例)和健康对照组23例。CVA治疗前组经规范化治疗3月后成CVA治疗后组(41例)。采用ELISA方法检测咳嗽变异性哮喘(CVA)患者血清中sPLA2-X、CysLT、iNOS、IL-33、IL-17、总IgE的含量;采用肺功能仪检测肺功能;呼出气一氧化氮检测仪检测FeNO。结果:与健康对照组相比,CVA治疗前组血清中sPLA2-X、CysLT、iNOS、IL-33的表达显著升高,差异具有统计学意义(P<0.05)。同时,CVA经规范治疗后,CVA治疗后组血清中sPLA2-X、CysLT、iNOS、IL-33及总IgE显著降低,FeNO明显降低、肺功能改善,差异均具有统计学意义(P<0.05)。而CVA治疗前组血清中IL-17与健康对照组相比,无显著差异。CVA治疗前组血清中sPLA2-X含量与CysLT、FeNO、iNOS、IL-33呈正相关(r=0.854 7、0.622 9、0.644 9、0.778 9,P<0.001),IL-17与血清中sPLA2-X含量无相关性(r=0.058 5,P=0.127 6)。结论:CVA患者为支气管哮喘特殊类型,其血清中sPLA2-X与CysLT、iNOS、IL-33均显著升高,同时FeNO显著升高。经治疗后血清中sPLA2-X、CysLT、iNOS、IL-33显著降低。血清中sPLA2-X含量与CysLT、iNOS、IL-33及FeNO呈正相关。
Objective:To analyze the relationship between sPLA2-X and CysLT,iNOS,IL-33 in serum and FeNO of patients with cough variant asthma(CVA),and further study the effect of sPLA2-X on CVA.Methods:The patients with firstly diagnosed CVA in outpatients from June 2018 to November 2019 in the Department of Respiratory and Critical Care Medicine of Guizhou Provincial People’s Hospital were selected as the case group,and the healthy patients who were examined in our hospital’s health examination center during the same period were selected as the control group. The patients in the case group were divided into two groups:the CVA group before treatment(41 cases)and the CVA group after treatment(41 cases),and healthy control group were 23 cases. The CVA group before treatment was standardized treated after 3 months was as the CVA group after treatment group(41 cases). ELISA was used to detect sPLA2-X,CysLT,iNOS,IL-33,IL-17 in the serum of patients with cough variant asthma(CVA). Lung function was examined,and FeNO was detected using an exhaled nitric oxide detector.Results:Compared with the healthy control group,the expressions of sPLA2-X,CysLT,iNOS and IL-33 in the serum of patients with CVA before treatment were significantly higher,and the differences were statistically significant(P<0.05).In addition,after standard treatment of CVA,sPLA2-X,CysLT,iNOS,IL-33 and total IgE in serum of patients with CVA group after treatment,which were significantly reduced,FeNO was significantly decreased,and lung function was improved than those in CVA group before treatment,the differences were statistically significant(P<0.05).However,there was no significant difference in IL-17 in serum CVA group before treatment compared with healthy controls. In the CVA group before treatment,the sPLA2-X content in serum was positively correlated with CysLT,FeNO,iNOS and IL-33 in serum(r=0.854 7,0.622 9,0.644 9,0.778 9,P<0.001). IL-17 was not correlated with SPLA2-X(r=0.058 5,P=0.127 6).Conclusion:CVA patients are special type of bronchia asthma. The level of SPLA2-X and CysLT,iNOS,IL-33,total IgE in serum were significantly increased,and FeNO was significantly increased. After treatment,the levels of sPLA2-X,CysLT,iNOS,IL-33 and total IgE in serum decreased significantly,and the levels of sPLA2-X were positively correlated with CysLT,iNOS,IL-33 and FeNO.
