摘要
目的探究小儿急性阑尾炎患儿实施尿常规检验与C反应蛋白(CRP)检验的诊断价值。方法100例疑似小儿急性阑尾炎患儿,术前均接受尿常规检验、CRP检验,比较CRP检验与尿常规检验的敏感度、特异度、准确度;比较两种诊断方式对不同类型急性阑尾炎的检出率;手术前后CRP水平变化及两种诊断方式诊断用时。结果CRP诊断敏感度91.76%(78/85)、特异度80.00%(12/15)、准确度90.00%(90/100)。尿常规检验诊断敏感度84.71%(72/85)、特异度66.67%(10/15)、准确度82.00%(82/100)。两种诊断方式的敏感度、特异度、准确度比较,差异无统计学意义(P>0.05)。CRP检验对单纯性阑尾炎的检出率94.34%高于尿常规检验的69.81%,差异有统计学意义(P<0.05)。术前,术后1、5、10 h,CRP水平分别为(60.52±12.66)、(37.62±10.85)、(24.63±8.91)、(13.63±5.62)mg/L,CRP水平术前>术后1 h>术后5 h>术后10 h,差异有统计学意义(P<0.05)。CRP诊断用时(6.32±2.15)h长于尿常规检验诊断的(1.83±1.21)h,差异有统计学意义(P<0.05)。结论在小儿急性阑尾炎诊断中尿常规检验、CRP检验均具有显著的诊断效果,CRP诊断特异度更高,可对患儿疾病类型、严重程度给予有效反映,但相比尿常规检验操作较为繁琐,用时较长,故需根据实际病情选择合适的诊断方式。
Objective To investigate the value of routine urine test and C-reactive protein(CRP)in the diagnosis of acute appendicitis in children.Methods There were 100 children with suspected acute appendicitis all received routine urine test and C-reactive protein test before operation.Comparison was made on sensitivity,specificity,accuracy of CRP test and routine urine test,and their detection rate for different types of acute appendicitis,changes in CRP levels before and after operation and the diagnostic time of the two diagnostic methods.Results The sensitivity,specificity and accuracy of CRP test were 91.76%(78/85),80.00%(12/15),and 90.00%(90/100).The sensitivity,specificity and accuracy of routine urine test were 84.71%(72/85),66.67%(10/15),and accuracy 82.00%(82/100).There was no statistically significant difference in sensitivity,specificity and accuracy between the two tests(P>0.05).The detection rate of CRP test for simple appendicitis was 94.34%,which was higher than 69.81%of routine urine test,and the difference was statistically significant(P<0.05).The CRP levels were(60.52±12.66),(37.62±10.85),(24.63±8.91),(13.63±5.62)mg/L before operation and 1,5,10 h after operation.The CRP level before operation>1 h after operation>5 h after operation>10 h after operation,and the difference was statistically significant(P<0.05).The diagnostic time of CRP was(6.32±2.15)h,which was longer than(1.83±1.21)h of routine urine test,and the difference was statistically significant(P<0.05).Conclusion Both routine urine test and C-reactive protein test have significant effect in the diagnosis of acute appendicitis in children.The specificity of CRP diagnosis is higher,which can effectively reflect the disease type and severity of children.However,compared with routine urine test,the operation is more complicated and takes longer time.Therefore,it is necessary to select the appropriate diagnosis method according to the actual condition.
