摘要
目的评价小剂量右美托咪定对脑功能区胶质瘤手术患者应激指标及清醒质量的影响。方法择期全麻下脑功能区胶质瘤手术患者73例,年龄34~46岁,BMI 24~28 kg/m 2,ASA分级Ⅱ~Ⅲ级。采用随机数字表法分为小剂量组(A组,24例)、中剂量组(B组,24例)及大剂量组(C组,25例)。麻醉诱导前三角肌内分别注射右美托咪定(批号:20011431,扬子江药业股份有限公司)0.5μg/kg、1μg/kg、1.5μg/kg。分别于入室时、唤醒后5 min及手术结束时检测患者的去甲肾上腺素、肾上腺素水平。采用本院自制“清醒质量表”评估患者的清醒质量。结果3组患者各时间点的去甲肾上腺素、肾上腺素水平差异均无统计学意义(P>0.05)。A组清醒质量评分高于B组、C组,差异有统计学意义(P<0.05)。结论麻醉诱导前三角肌内注射不同剂量的右美托咪定均可减轻脑功能区胶质瘤手术患者的应激反应,其中小剂量右美托咪定可提高患者的清醒质量。
Objective To investigate the effect of low-dose dexmedetomidine on the stress indicators and the quality of wakefulness in patients with glioma surgery of the brain functional area.Methods Choose 73 patients with glioma surgery,age 34~46 years old,BMI 24~28 kg/m ^(2),ASA grade.By the randomized digital table method they were divided into small dose group(A group,24 cases),medium dose group(B group,24 cases),and large dose group(C group,25 cases).Aninduced preinjection(batch number:20011431,Yangtze River Pharmaceutical Co.Ltd.0.5μg/kg,1μg/kg,1.5μg/kg.)Norepinephrine and adrenaline levels were detected at operation room,5 min after wake-up,and at the end of the surgery,respectively.The sober quality table self-made by the hospital was used to evaluate the awake quality of patients.Results There was no statistical difference in norepinephrine and epinephrine levels among the 3 group patients(P>0.05).The sober mass score was higher in A than B and C,and the differences were statistically significant(P<0.05.Conclusion Anesthesia-induced injection of different doses of dextrotomidine can reduce the stress response of patients with glioma surgery in the brain function area,but small doses of dextrotomidine can improve the awake quality of patientst.
