摘要
目的探讨通络解痉汤联合推拿治疗中风后偏瘫痉挛状态的疗效及对日常生活能力影响。方法选取医院收治的中风后偏瘫痉挛状态患者90例,随机分为两组各45例。对照组给予推拿治疗,治疗组在对照组基础上给予通络解痉汤,对比治疗后临床疗效、Fugl-Meyer评分、Barthel指数评分和CSS评分及上肢和下肢Ashworth分级及SF-36量表评分。结果治疗组总有效率较高(P<0.05);两组患者治疗前Fugl-Meyer评分、Barthel指数评分和CSS评分差异无统计学意义(P>0.05);治疗后两组患者CSS评分明显降低,Fugl-Meyer评分和Barthel指数评分明显升高(P<0.05);且治疗组改善较明显(P<0.05);两组患者治疗前上肢和下肢Ashworth分级差异无统计学意义(P>0.05);两组患者治疗后上肢和下肢Ashworth分级明显降低(P<0.05);且治疗组降低较多(P<0.05);两组患者治疗前SF-36量表评分差异无统计学意义(P>0.05);两组患者治疗后SF-36量表各条目评分显著升高(P<0.05);并且实验组升高较多(P<0.05);两组患者治疗前中医证候评分无差异(P>0.05);治疗后两组患者中医证候评分明显降低(P<0.05);且治疗组降低较明显(P<0.05)。结论采用通络解痉汤联合推拿治疗中风后偏瘫痉挛状态具有较好的治疗效果,能够改善患者日常生活能力,值得在临床上推广应用。
Objective To investigate the effect of Tongluo Jiejing Decoction(通络解痉汤) combined with Tuina on hemiplegia spasmodic state after stroke and its effect on the ability of daily living. Methods Ninety cases of hemiplegia spasticity patients after stroke admitted to our hospital were randomly divided into two groups, 45 cases in each. The control group was given Tuina treatment, and the treatment group was given Tongluo Jiejing Decoction on the basis of the control group. The clinical efficacy, fugl-meyer score, Barthel index score, CSS score, Ashworth grading of upper and lower limbs, and SF-36 scale score after treatment were compared. Results The total effective rate was higher in the treatment group(P<0.05). There was no difference in fugl-meyer score, Barthel index score or CSS score between the two groups before treatment(P>0.05). After treatment, the CSS score of the two groups was significantly reduced, and the fugl-meyer score and Barthel index score were significantly increased(P<0.05). The improvement in the treatment group was significant(P<0.05). There was no difference in Ashworth grading of upper and lower limbs between the two groups before treatment(P>0.05). The Ashworth grading of upper and lower limbs was significantly reduced after treatment in the two groups(P<0.05). There was a significant decrease in the treatment group(P<0.05). There was no difference in SF-36 scale score between the two groups before treatment(P>0.05). The scores of SF-36 items in the two groups were increased significantly after treatment(P<0.05). There was a significant increase in the treatment group(P<0.05). There was no difference in TCM syndrome score between the two groups before treatment(P>0.05). After treatment, TCM syndrome score of patients in the two groups was significantly reduced(P<0.05). The decrease was significant in the treatment group(P<0.05). Conclusion Tongluo Jiejing Decoction combined with Tuina for hemiplegia spasmodic state after stroke has a good therapeutic effect and can improve the patients’ daily living ability, worthy of clinical application.
