摘要
目的探讨阿胶鸡子黄汤加减治疗青光眼阴虚火旺证的疗效以及对血清基质金属蛋白酶(MMP)-2和金属蛋白酶组织抑制因子(TIMP)-2水平的影响。方法选择2017年1月—2019年9月医院收治的开角型青光眼患者76例,按随机数字表法分为对照组和观察组每组38例。对照组接受马来酸噻吗洛尔滴眼液,早晚各1次,每次1滴,连续治疗3个月。观察组在对照组治疗基础上给予阿胶鸡子黄汤加减治疗,每日1剂,2次/d,连续治疗3个月。比较两组最佳矫正视力、眼压、临床疗效、视网膜中央动脉血流参数以及血清MMP-2和TIMP-2水平。结果治疗后,观察组患者的最佳矫正视力和眼压显著低于对照组(P<0.01);观察组总有效率(94.74%,36/38)显著高于对照组(73.68%,28/38)(P<0.05);治疗后,观察组患者视网膜中央动脉收缩期峰值血流速度(PSV)、血流的舒张末期血流速度(EDV)明显高于对照组,血管阻力指数(RI)显著低于对照组(P<0.01);治疗后,观察组患者血清MMP-2水平明显高于对照组,TIMP-2水平显著低于对照组(P<0.01)。结论阿胶鸡子黄汤加减可有效提高青光眼阴虚火旺证的临床疗效,改善患者的视力和眼压,调节血清MMP-2、TIMP-2水平。
Objective To investigate the efficacy of Ejiao Jizihuang decoction(阿胶鸡子黄汤) treating glaucoma with syndrome of Yin deficiency and fire excess and its effect on serum levels of matrix metalloproteinase(MMP)-2 and metalloproteinase tissue inhibitor(TIMP)-2.Methods Seventy-six open-angle glaucoma admitted to the hospital from January 2017 to September 2019 were selected and divided into control group and observation group(38 cases in each group) according to the method of random number table. The control group received timolol maleate eye drops with one time in the morning and one time in the evening for 3 months. The observation group was given Ejiao Jizihuang Decoction 1 dose, 2 times per day for 3 months on the basis of the treatment of the control group. The best corrected visual acuity, intraocular pressure, the clinical efficacy, blood flow parameters in the central retinal artery and serum levels of MMP-2 and TIMP-2 were compared between the two groups. Results After treatment, the best corrected visual acuity and intraocular pressure in the observation group were significantly lower than those in the control group(P<0.01). The total effective rate(94.74%,36/48) of the observation group was significantly higher than that of the control group(73.68%,28/38)(P<0.05). After treatment, the central retinal artery systolic peak flow velocity(PSV) and end-diastolic flow velocity(EDV) in the observation group were significantly higher, while the vascular resistance index(RI) was significantly lower than that in the control group(P<0.01). After treatment, the serum MMP-2 level of the observation group was significantly higher and TIMP-2 was significantly lower than those of the control group(P<0.01). Conclusion In the treatment of glaucoma with syndrome of Yin deficiency and fire excess, Ejiao Jizihuang Decoction can increase the efficacy, improve the visual acuity and intraocular pressure and regulate serum levels of MMP-2 and TIMP-2.
