摘要
目的:探讨不同入路全髋关节置换术对患者术后再手术和脱位影响的相关性。方法:选取2019年4月-2020年4月我院收治行全髋关节置换术的104例股骨颈骨折患者,根据随机数字表分为对照组52例与观察组52例,对照组患者予后外侧入路全髋关节置换术,观察组予直接外侧入路全髋关节置换术,术后随访12个月,比较2组患者术后再手术率与脱位率,并通过Cox比例风险回归模型分析全髋关节置换术后出现脱位与再手术的影响因素。结果:观察组患者再手术率15.38%与术后脱位率3.85%,均明显低于对照组30.77%,13.46%(P<0.05),但2组患者术后假体周围骨折、深部感染、假体松动发生率比较(P>0.05);Cox比例风险回归模型评估显示后外侧入路是全髋关节置换术后脱位与再手术发生的影响因素(P<0.05)。结论:不同入路全髋关节置换术治疗后再手术和脱位发生率不同,对股骨颈骨折患者采取后外侧入路全髋关节置换术治疗股骨颈骨折,较直接外侧入路全髋关节置换术出现再手术和脱位的概率更高。
Objective:To investigate the influence of different approaches of total hip arthroplasty on postoperative reoperation and dislocation.Methods:104 patients with femoral neck fracture who underwent total hip arthroplasty in our hospital from September 2018 to September 2019 were selected and divided into control group(52 cases)and experimental group(52 cases)according to the random number table.The patients in the control group were treated with total hip arthroplasty through posterolateral approach,and the experimental group were treated with direct lateral total hip arthroplasty Cox proportional hazard regression model was used to analyze the influencing factors of dislocation and reoperation after total hip arthroplasty.Results:The reoperation rate of 15.38%and postoperative dislocaion rate of 3.85%in the experimental group were significantly lower than 30.77%and 13.46%in the control group(P<0.05),but the incidence of periprosthetic fracture,deep infection and prosthesis loosening in the two groups were compared(P>0.05)The proportional risk regression model showed that posterolateral approach was the influencing factor of dislocation and reoperation after total hip arthroplasty(P<0.05).Conclusion:The incidence of reoperation and dislocation is different after total hip arthroplasty through different approaches.The probability of reoperation and dislocation in patients with femoral neck fracture treated by posterolateral total hip arthroplasty is higher than that of direct lateral total hip arthroplasty.
