摘要
目的利用疾病诊断相关分组(DRGs)指标对某综合医院骨科医疗服务现状进行绩效评价,为医院推进DRGs应用和加强医疗质量管理提供帮助。方法应用上海联众DRGs分组器处理某综合医院骨科2018年1月1日-2019年12月31日住院病例病案首页数据,采用综合指数法对指标进行指数处理并评价。结果研究医院骨科2019年较2018年CMI平均上升0.0294,DRG组数平均增加21组,费用消耗指数平均下降0.03,时间消耗指数平均减少0.04,两年低风险组死亡率皆为0%。骨科A5病区综合指数2018年及2019年均排名第一。结论2018-2019年该医院骨科医疗服务能力和服务效率有所提升,科室能够收治大部分骨科疾病,而骨科各病区发展具有一定的差异,须注意在保证服务效率和安全的同时,增加疑难病例的收治,并控制患者住院费用和住院时间。
Objective To evaluate the current situation of orthopedic medical service in a general hospital by using diagnosis related groups(DRGs)index,and to provide help for promoting the application of DRGs and strengthening medical quality management.Methods Shanghai Lianzhong DRGS Grouper was used to process the first page data of inpatient cases in the Orthopedics Department of a general hospital from January 1st,2018 to December 31st,2019.Comprehensive index method was used to process and evaluate the indexes.Results Compared with in 2018,case mix index(CMI)increased by 0.0294 averagely in 2019,the number of DRG groups increased by 21 averagely,the cost consumption index decreased by 0.03 averagely,and the time consumption index decreased by 0.04 averagely.The two year mortality rate of the low risk group was 0%.Orthopedics A5 Ward Comprehensive Index ranked the first from 2018 to 2019.Conclusion From 2018 to 2019,the orthopedic medical service capacity and service efficiency of the hospital have been improved.Most orthopedic diseases can be treated in the departments of the hospital.However,the development of each orthopedic ward has certain differences.
