摘要
目的研究外周血特异性肿瘤标志物mRNA表达在非小细胞肺癌(NSCLC)微转移中的意义。方法采用逆转录聚合链反应(RT-PCR)技术对江西省胸科医院病理科2019年1月—2020年11月收治的52例NSCLC患者与30例正常体检者(参照组)的外周血细胞角蛋白(CK19)、肺特异性X基因(LUNX)、癌胚抗原(CEA)mRNA表达进行检测,并分析NSCLC患者微转移标志物与病理类型、分化程度及临床分期的关系。结果NSCLC组的CK19、LUNX、CEA mRNA的阳性率分别为38.46%、57.69%、44.23%,均高于参照组(均为0),差异有统计学意义(P<0.05)。Ⅲ期NSCLC患者的LUNX mRNA的阳性率均高于Ⅰ期与Ⅱ期患者,差异有统计学意义(P<0.0167)。高分化NSCLC患者的LUNX mRNA的阳性率低于低、中分化患者,差异有统计学意义(P<0.0167)。结论LUNX mRNA表达的阳性率随NSCLC患者TNM分期增高而增加,随分化程度升高而降低,与NSCLC的微转移有密切关系,可作为NSCLC患者微转移检测的分子标志物,对早期确诊肺癌转移、指导临床分期与治疗有重要意义。
Objective To investigate the significance of mRNA expression of specific tumor markers in peripheral blood in micro-metastasis of non-small cell lung cancer(NSCLC).Methods The mRNA expressions of CK19,LUNX and CEA in peripheral blood of 52 NSCLC patients and 30 normal subjects(the reference group)who were admitted to the Department of Pathology of Jiangxi Chest Hospital from January 2019 to November 2020 were detected by reverse transcription polymerase chain reaction(RT-PCR)technique.The relationship between micro-metastatic markers and pathological types,degree of differentiation and clinical stages in NSCLC patients was analyzed.Results The mRNA positive rates of CK19,LUNX and CEA in NSCLC group were 38.46%,57.69%and 44.23%,respectively,which were higher than those in the control group(0.00%,0.00%and 0.00%),and the differences were statistically significant(P<0.05).The positive rate of LUNX mRNA in stageⅢof NSCLC patients was higher than that of the patients in stageⅠandⅡ,and the difference was statistically significant(P<0.0167).The positive rate of LUNX mRNA in highly differentiated NSCLC patients was lower than that in poorly differentiated and moderately differentiated NSCLC patients,and the difference was statistically significant(P<0.0167).Conclusion The positive rate of LUNX mRNA expression increases with the increase of TNM stage in NSCLC patients,and decreases with the increase of differentiation degree,which is closely related to the micro-metastasis of NSCLC patients,and can be used as a molecular marker for the detection of micro-metastasis in NSCLC patients,which is of great significance for the early diagnosis of lung cancer metastasis and the guidance of clinical staging and treatment.
