摘要
目的:观察大承气汤凝胶贴片经超声穴位导入治疗慢阻肺无创通气患者并发肠梗阻的临床疗效。方法:将我科68例慢阻肺患者经无创通气治疗后发生肠梗阻的患者随机分为对照组和实验组。对照组予常规治疗,实验组在常规治疗基础上予大承气汤凝胶贴片联合穴位经超声导入治疗,每日1次,连续治疗7天为1个疗程,共观察1个疗程。观察患者治疗后首次排气及排便时间、胃肠症状(GSRS)“腹胀”评分、动脉血氧分压(PaO_(2))及动脉血二氧化碳分压(PaCO_(2))参数、日常生活能力(ADL)评分。结果:实验组治疗后初次排气时间和初次排便时间显著短于对照组,胃肠症状(GSRS)腹胀评分显著低于对照组(P<0.01);患者动脉血氧分压(PaO2)明显升高,动脉血二氧化碳分压(PaCO2)明显降低,日常生活能力(ADL)评分明显升高,患者无创通气治疗效果及日常活动能力改善明显优于对照组(P<0.05)。结论:大承气汤凝胶贴片联合穴位经超声导入治疗对慢阻肺无创通气患者并发肠梗阻的效果显著,值得推广。
Objective:To observe the efficacy of Dachengqi Decoction gel patch by ultrasound acupoint introduction treatment on intestinal obstruction foRpatients with chronic obstructive pulmonary noninvasive ventilation.Methods:The 68 patients with COPD in ouRdepartment who had intestinal obstruction afteRnon-invasive ventilation treatment were randomly divided into a controlled group and an experimental group.The controlled group receives the conventional therapy,and the test group was given the treatment of Dachengqi Decoction gel patch combined with acupoint ultrasonic conductivity once daily continuously foR7 days,which constituted 1 treatment cycle.AfteR1 treatment cycle,we observed several important indicators of the patients such as the post-treatment time to first annual exhaust and to first defecation,abdominal distension score on the gastrointestinal symptom rating scale(GSRS),arterial oxygen partial pressure(PaO2),arterial carbon dioxide partial pressure(PaCO_(2)),and activities of daily living(ADL)score.Results:Compared to the controlled group,the test group achieved significantly shorteRtime in first annual exhaust and first defecation,and obtained the significantly loweRabdominal distension score on GSRS(P<0.01).Meanwhile,in the test group,PaO2 was significantly increased but PaCO_(2) was significantly decreased.Besides,the ADL score on the therapeutic effect of noninvasive ventilation and improvement was significantly betteRin contrast with the controlled group(P<0.05).Conclusion:Dachengqi Decoction gel patch combined with acupoint ultrasonic conductivity can achieve significant efficacy in the treatment of intestinal obstruction caused by noninvasive ventilation foRpatients with COPD and it is worth exploring its furtheRclinical application.
