超声造影灌注时相分析对肝硬化结节、异型增生结节及微小肝细胞癌的早期诊断价值 被引量:6

摘要 目的分析实时超声造影灌注时相分析对肝硬化结节、异型增生结节及微小肝细胞癌的早期诊断价值。方法选取2015年1月至2019年11月开封市中医院收治的132例定期接受超声检查的肝硬化伴肝内异常结节(295枚肝内结节)患者。参照影像学检查和病理检查的标准,采用实时超声造影灌注时相分析患者不同性质结节增强表现的特点。结果295枚肝内结节中,肝硬化结节207枚,异型增生结节25枚,微小肝细胞癌(肿瘤最大径不超过2 cm)63枚。不同结节类型的患者超声灰阶声像图特征等回声和高回声的占比比较,差异有统计学意义(P<0.05);不同结节类型的患者超声灰阶声像图特征低回声占比比较,差异无统计学意义(P>0.05)。不同类型肝内结节患者的超声造影增强方式比较,差异有统计学意义(P<0.05)。实时超声造影早期诊断肝硬化结节、异型增生结节及微小肝细胞癌的准确率分别为93.24%、80.00%、87.30%。不同类型肝内结节准确率比较,差异无统计学意义(P>0.05)。结论采用实时超声造影灌注时相分析可客观、动态显示肝硬化结节、异型增生结节及微小肝细胞癌的不同血流动力学灌注特征。通过定期监测肝硬化结节的进展情况可对微小肝细胞癌的发生进行及时干预和早期诊断。 Objective To analyze the early diagnostic value of real-time contrast-enhanced ultrasound perfusion phase analysis for liver cirrhosis nodules,dysplastic nodules and micro-hepatocellular carcinoma.Methods From January 2015 to November 2019,132 patients with liver cirrhosis and abnormal liver nodules(295 intrahepatic nodules)admitted to Kaifeng Hospital of Traditional Chinese Medicine who underwent regular ultrasound examinations were selected.According to the standards of imaging examination and pathological examination,real-time contrast-enhanced ultrasound perfusion phase was used to analyze the characteristics of the enhancement performance of patients with different types of nodules.Results Among 295 intrahepatic nodules,207 were cirrhotic nodules,and 25 were dysplastic nodules,and 63 were micro-hepatocellular carcinoma(the largest diameter of tumor was no more than 2 cm).Comparison of the proportions of iso-echo and hyperechoic characteristics of ultrasound gray-scale sonographic features of patients with different nodule types was statistical different(P<0.05).There was no statistical difference in the proportion of hypoechoic characteristics in the gray-scale ultrasound images of patients with different nodule types(P>0.05).There was a statistical difference in contrast enhancement methods of patients with different types of intrahepatic nodules(P<0.05).The accuracy rates of real-time contrast-enhanced ultrasound in the early diagnosis of cirrhotic nodules,dysplastic nodules and micro-hepatocellular carcinoma were 93.24%,80.00% and 87.30%,respectively.There was no statistical difference in the accuracy of different types of intrahepatic nodules(P>0.05).Conclusion The real-time contrast-enhanced ultrasound perfusion phase analysis could objectively and dynamically display the different hemodynamic perfusion characteristics of cirrhotic nodules,dysplastic nodules and micro-hepatocellular carcinoma.Regular monitoring of the progress of liver cirrhosis nodules can promptly intervene and diagnose the occurrence of micro-hepatocellular carcinoma.
