Application Value of Magnetic Resonance Imaging in the Diagnosis of Prenatal Placenta Accrete

摘要 目的:探讨磁共振成像在诊断产前胎盘植入的应用价值。方法:回顾性分析2019年6月—2020年6月在我院产前就诊的40例疑似PIA患者的产前MRI检查资料,以手术病理结果为"金标准",分析胎盘植入患者的MRI图像特点,并采用Kappa探讨MRI诊断与病理"金标准"的一致性。结果:40例疑似患者中经病理确诊为PIA的患者为32例,32例均可见全部或部分胎盘与肌层分界模糊,分界线不连续,其中20例可见子宫胎盘结合面信号中断及子宫局部膨隆;MRI对于PIA总体及其3种分级(粘连型、肌层植入型、穿透型)辅助诊断的敏感度分别为93.8%、44.4%、95.0%、66.7%;特异度分别为87.5%、96.8%、100.0%、100.0%;Kappa值分别为0.850、0.565、0.704、0.597。结论:磁共振成像应用于胎盘植入的诊断中具有较高的诊断效能,可为临床诊断提供良好依据。 Objective:To investigate the application value of magnetic resonance imaging in the diagnosis of prenatal placenta accrete.Methods:Retrospective analysis of 40 cases of suspected PIA patients in our hospital from June 2019 to June 2020,the diagnostic efficiency of MRI for PIA and the detection rate of MRI imaging features were analyzed,Kappa analysis was used to investigate the consistency between MRI diagnosis and pathological"gold standard".Results:Among 40 subjects,32 of them were confirmed as PIA by results of histopathological examination after operation,in all 32 cases,the boundary between the placenta and the muscular layer was blurred and discontinuous,in 20 cases,the signal of the interface between uterus and placenta was interrupted and uterine swelling was found.The sensitivity of MRI in the diagnosis of PAS and its three grades were as follows:93.8%、44.4%、95.0%、66.7%;The specificity were:87.5%、96.8%、100.0%、100.0%;Kappa values were 0.850、0.565、0.704、0.597.Conclusion:Magnetic resonance imaging in the diagnosis of placenta accrete has high diagnostic efficiency and can provide a good basis for clinical diagnosis.
