摘要
目的探究血清硫脑苷脂、肾小球滤过率(eGFR)与老年冠心病(CHD)患者左心结构和功能的关系。方法选取2017年1月至2020年3月本院收治的CHD患者80例,根据eGFR水平分为A组(n=23)、B组(n=33)与C组(n=24)。比较3组患者年龄、性别、体重指数(BMI)、血清硫脑苷脂水平、eGFR水平、心结构与心功能指标以及高血压、糖尿病病史等指标。分析血清硫脑苷脂、eGFR与CHD患者左心结构与功能指标的关系。结果3组年龄、血清硫脑苷脂、eGFR、LA、ESD、EDD、EF、FS、LVM、E峰以及ALB、BUN、SCr、K、Ca、P、MAP、DBP、SBP比较差异有统计学意义(P<0.05)。未调整变量前血清硫脑苷脂与LA呈负相关,与eGFR呈正相关,eGFR与LA呈负相关;多变量调整后,eGFR与血清硫脑苷脂呈正相关,与E峰呈负相关,差异均有统计学意义(P<0.05)。结论老年CHD患者血清硫脑苷脂与eGFR相关,且二者与患者部分心功能指标相关,二者联合检查可预防和缓解心功能不全。
Objective To analyze the relationship between serum sulfatide,glomerular filtration rate(eGFR)and left heart structure and function in elderly coronary heart disease(CHD).Methods 80 patients with CHD from January 2017 to March 2020 in our hospital were selected.They were divided into group A(n=23),group B(n=33)and group C(n=24)according to eGFR level.The age,sex,body mass index(BMI),history of hypertension,history of diabetes,serum sulfatide level,eGFR level,cardiac structure and cardiac function indexes and other indexes were compared among the three groups.The relationship between serum thiocephalin,eGFR and left ventricular structure and functional indexes in patients with CHD was analyzed.Results There was significant difference inn age,serum thiocerebral glucoside,EGFR,LA,ESD,EDD,EF,FS,LVM,E peak,ALB,BUN,SCR,K,Ca,P,MAP,DBP,SBP among three groups(P<0.05).Before variable adjustment,serum sulfatide was negatively correlated with LA and positively correlated with eGFR;eGFR was negatively correlated with LA.After multivariate adjustment,eGFR was positively correlated with serum sulfatide and negatively correlated with E peak,the difference was statistically significant(P<0.05).Conclusion Serum sulfatide is related to eGFR in elderly patients with CHD,and both are related to some cardiac function indexes of the patients.Combining the two examinations have important value in preventing and alleviating cardiac dysfunction.
