摘要
目的探讨急性缺血性脑卒中院内合并肺部感染的相关危险因素。方法选择2018年5月至2020年5月广东省云浮市人民医院神经内科收治的71例急性缺血性脑卒中院内合并肺部感染患者为感染组,另选同期收治的无肺部感染急性缺血性脑卒中患者71例为对照组,对两组患者的相关危险因素进行分析。结果经单因素分析显示,感染组性别、是否合并高血压、是否合并糖尿病、是否静脉溶栓与对照组比较,差异无统计学意义(P>0.05),感染组年龄≥70岁、NIHSS评分>15分、GCS评分≤8分、伴吞咽功能障碍、气管插管>1 d、呼吸道辅助通气>3 d的占比明显高于对照组,差异有统计学意义(P<0.05);经logistic多因素分析显示,年龄≥70岁、NIHSS评分>15分、GCS评分≤8分、伴吞咽功能障碍、气管插管>1 d、呼吸道辅助通气>3 d是急性缺血性脑卒中院内合并肺部感染的独立危险因素。结论年龄、病情严重程度、气管插管、伴吞咽功能障碍等是导致急性缺血性脑卒中患者出现院内肺部感染的主要危险因素。
Objective To investigate the related risk factors of nosocomial pulmonary infection in patients with acute ischemic stroke(AIS).Methods A total of 71 AIS patients with nosocomial pulmonary infection admitted to our hospital from May 2018 to May 2020 were selected as the infection group,and another 71 AIS patients without pulmonary infection admitted in the same period were selected as the control group.The related risk factors of patients in the two groups were analyzed.Results The univariate analysis showed that there were no statistically significant differences between the infection group and the control group in terms of gender,presence or absence of combined hypertension,presence or absence of combined diabetes and intravenous thrombolysis(P>0.05).The proportions of age≥70 years,score of the National Institute of Health Stroke Scale(NIHSS)>15,score of the Glasgow Coma Scale(GCS)≤8,with swallowing dysfunction,tracheal intubation>1 d,and respiratory assisted ventilation>3 d in the infection group were significantly higher than those in the control group,with statistically significant differences(P<0.05).The Logistic-multivariate analysis showed that age≥70 years,NIHSS>15,GCS≤8,with swallowing dysfunction,tracheal intubationz>1 d,and respiratory assisted ventilation>3 d were the independent risk factors for AIS patients with nosocomial pulmonary infection.Conclusion Age,severity of disease,tracheal intubation,and swallowing dysfunction are the main risk factors for the oc currence of nosocomial pulmonary infections in AIS pat ients.
