摘要
目的探讨胃泌素17(G-17)、胃蛋白酶原(PG)、CA724、CA242血清标志物联合检测对老年胃癌的诊断价值。方法选择2018年1月至2019年12月来山东省乳山市中医院诊治和体检的老年人作为研究对象,根据胃镜及病理检查结果,将研究对象分为三组:胃癌组、胃良性病变组、健康对照组,每组各50例,分别检测G-17、PGⅠ、PGⅡ、CA724、CA242水平并进行ROC分析。结果胃癌组患者血清G-17、CA724和CA242水平均显著高于胃良性病变组和对照组(P<0.05),PGⅠ、PGR水平则低于其它两组;血清标志物单独检测时,PGⅠ/PGⅡ(PGR)的AUC、灵敏度最大,分别为0.89和88.0%,G-17特异度最高为81.0%;五种标志物串联时特异度最高(89.0%),并联时灵敏度最高(92.0%)。结论血清标志物联合检测能够有效提高老年胃癌患者诊断的准确性,具有重要的临床诊断和应用价值。
Objective To investigate the diagnostic value of combined detection of gastrin 17(G-17),pepsinogen(PG),carbohydrate antigen CA724 and CA242 serum markers in elderly patients with gastric cancer.Methods The elderly admitted to Rushan Traditional Chinese Medicine Hospital for diagnosis,treatment and physical examination from January 2018 to December 2019 were selected as the researc h objects.According to the results of gastroscopy and pathological examination,the research objects were divided into the gastric cancer group(n=50),the benign gastric lesions group(n=50)and the healthy control group(n=50).The levels of G-17,PGⅠ,PGⅡ,CA724 and CA242 were detected and analyzed by receiver operating characteristic curve(ROC).Results The serum levels of G-17,CA724 and CA242 in patients of the gastric cancer group were significantly higher than those of the benign gastric lesions group and the control group(P<0.05).Meanwhile,the levels of PGⅠand PGR were lower than those in other two groups.When serum markers were detected alone,the AUC and sensitivity of PGⅠ/PGⅡ(PGR)were the highest,which were 0.89 and 88.0%,respectively.And the highest specificity of G-17 was 81.0%.The specificity of the five markers was the highest(89.0%)in series,while the sensitivity was the highest(92.0%)in parallel.Conclusion The combined detection of serum markers can effectively improve the diagnostic accuracy of elderly patients with gastric cancer,and it has important values in clinical diagnosis and a pplication.
