基于FibroTouch的非酒精性脂肪性肝病的临床分析

摘要 目的:分析非酒精性脂肪性肝病(nonalcoholic fatty liver disease,NAFLD)基于FibroTouch的临床价值。方法:回顾性收集2016年6月—2019年6月期间在南通市第三人民医院住院、门诊NAFLD患者的个人信息、生化指标、基于FibroTouch检查的肝脏弹性值(liver stiffness measurement,LSM)及脂肪衰减值(fat attenuation parameters,FAP)等相关资料,分析各种指标之间的相关性。结果:FibroTouch检测诊断为肝纤维化、脂肪化有效率高。LSM与FAP之间不存在明显关联性。无纤维化组的丙氨酸氨基转移酶(alanine aminotransferase,ALT)、天冬氨酸转氨酶(aspartate aminotransferase,AST)、低密度脂蛋白胆固醇(low density lipoprotein cholesterol,LDL-C)、体质量指数(body mass index,BMI)均较纤维化组低(均P<0.05);无脂肪化组的ALT、AST、总胆固醇(total cholesterol,TC)、三酰甘油(triglyceride,TG)、LDL-C、空腹血糖、BMI指标均较脂肪化组低(均P<0.05)。FibroTouch检测LSM值与ALT、AST、BMI呈正相关(rs>0,P<0.05),与肌酐呈负相关(rs<0,P<0.05);FAP值与ALT、AST、TC、TG、LDL-C、BMI呈正相关(rs>0,P<0.05),与HDL-C呈负相关(rs<0,P<0.05),且与BMI的相关系数最大。结论:应用FibroTouch可以判断NAFLD患者的肝脏纤维化、脂肪化病变程度,联合生化指标能更准确地诊断脂肪肝,有利于积极干预病情进展。 Objective:To analyze the clinical value of nonalcoholic fatty liver disease(NAFLD)based on FibroTouch.Methods:Personal information,biochemical indexes,liver stiffness measurement(LSM)and fat attenuation parameters(FAP)based on FibroTouch examination of patients with NAFLD in the Third People′s Hospital of Nantong From June 2016 to June 2019 were collected retrospectively to analyze the correlation among various indexes.Results:FibroTouch can efficiently detect and diagnose liver fibrosis and adipocytosis.There is no significant correlation between LSM and FAP.Alanine aminotransferase(ALT),aspartate aminotransferase(AST),low density lipoprotein cholesterol(LDL-C)and body mass index(BMI)in the non-fibrosis group were also lower than those in the fibrosis group,the difference was statistically significant(P<0.05).ALT,AST,total cholesterol(TC),triglyceride(TG),LDL-C,fasting blood glucose and BMI in the non-adipose group were lower than those in the adipose group,and the difference was statistically significant(P<0.05).LSM value detected by FibroTouch was positively correlated with ALT,AST and BMI(rs>0,P<0.05),inversely correlated with creatinine(rs<0,P<0.05).FAP was positively correlated with ALT,AST,TC,TG,LDL-C and BMI(rs>0,P<0.05),negatively correlated with HDL-C(rs<0,P<0.05),the strongest correlation was found with BMI.Conclusion:The application of FibroTouch can determine the degree of liver fibrosis and fatty lesions in patients with NAFLD.Fatty liver can be diagnosed more accurately combined with biochemical indicators.It is beneficial to active intervene the disease progress.
