摘要
目的探究艾司氯胺酮在改良电休克治疗中的应用效果。方法选取2019年1月至2020年5月本院精神科收治的100例患者作为研究对象,随机分为两组,各50例。所有患者均行改良电休克(MECT)治疗,对照组采用丙泊酚+氯化琥珀胆碱麻醉,艾司氯胺酮组在对照组基础上联合艾司氯胺酮麻醉,比较两组麻醉前、麻醉后、放电抽搐时、抽搐后的生命体征(血压、心率、血氧饱和度值)、不良反应发生率和苏醒时间。结果两组患者改良电休克治疗MECT均顺利完成。麻醉后,对照组血压和心率明显低于麻醉前(P<0.05),艾司氯胺酮组血压、心率明显高于对照组(P<0.05);放电抽搐时,两组血压、心率比较差异无统计学意义,但均明显高于麻醉前(P<0.05)。艾司氯胺酮组不良反应发生率为6.00%,明显低于对照组的20.00%(P<0.05)。艾司氯胺酮组苏醒时间为(4.88±0.79)min,短于对照组的(6.01±1.22)min,差异无统计学意义。结论艾司氯胺酮在改良电休克治疗中不仅能降低不良反应发生率,促进患者快速苏醒,在麻醉过程中还能稳定患者的各项生命体征,减少机体应激反应,值得临床推广应用。
Objective To explore the effect of esketamine in modified electroconvulsive therapy.Methods 100 patients admitted to the psychiatric department of our hospital from January 2019 to May 2020 were selected as the research subjects,and they were randomly divided into two groups,with 50 cases in each group.All patients underwent modified electroshock(MECT)treatment,the control group was anesthetized with propofol+succinylcholine chloride,and the esketamine group was anesthetized with esketamine on the basis of the control group.The vital signs(blood pressure,heart rate,blood oxygen saturation value),adverse reaction rate and recovery time before anesthesia,after anesthesia,during discharge convulsion and after convulsion were compared between the two groups.Results The modified electroconvulsive treatment between the two groups of patients was successfully completed.The blood pressure and heart rate of the control group after anesthesia were significantly lower than those before anesthesia(P<0.05).The blood pressure and heart rate in the esmketamine group were significantly higher than those in the control group(P<0.05);during the discharge twitch,there was no significant difference in the blood pressure and heart rate between the two groups,but both groups were significantly higher than before anesthesia(P<0.05).The incidence of adverse reactions in the esketamine group was 6.00%,which was significantly lower than 20.00%in the control group(P<0.05).The awakening time of the esketamine group was(4.88±0.79)min,which was shorter than(6.01±1.22)min in the control group,but the difference was not statistically significant.Conclusion Esketamine can not only reduce the incidence of adverse reactions in patients with modified electroconvulsive therapy and promote their rapid recovery,but also stabilize the vital signs of patients during anesthesia and reduce the body’s stress response.It is worthy of clinical application.
