摘要
目的对十二指肠乳头旁憩室(PAD)与胆总管结石的关系和其经内镜逆行胰胆管造影术(ERCP)取石的效果进行探究。方法随机选取该院于2018年7月—2020年6月接受ERCP取石的200例患者作为该次研究对象,将200例患者按照是否发生PAD分为两组,即PAD组(100例)和非PAD组(100例);对两组患者的临床资料以及与胆总管结石的关系和其ERCP取石的效果进行研究。结果不同PAD与十二指肠乳头相对位置患者胆总管结石发生率比较,边缘型(91.67%)>并列型(55.56%)>包绕型(50.00%),组间差异有统计学意义(P<0.05);但PAD组(99.00%)和非PAD组(98.00%)的取石成功率对比,差异无统计学意义(χ^(2)=0.000,P=1.000)。结论PAD对于ERCP取石的效果影响较小;只要医护人员掌握熟练的操作技巧,该种取石术对于PAD合并胆总管结石患者的治疗便具有一定的安全和有效性。
Objective To explore the relationship between parapapillary duodenal diverticulum(PAD)and choledocholithiasis and its effect on endoscopic retrograde cholangiopancreatography(ERCP)stone removal.Methods 200 patients in the hospital who underwent endoscopic retrograde cholangiopancreatography between July 2018 and June 2020 were selected random as the main subjects of this study.The 200 patients were selected according to whether the duodenal papilla diverticulum occurred and divided into two groups,namely:PAD group(100 cases)and non-PAD group(100 cases);the clinical data of the two groups of patients and the relationship with common bile duct stones and their effects on stone removal by endoscopic retrograde cholangiopancreatography was studied.Results Comparison of the incidence of common bile duct stones in patients with different relative positions of PAD and duodenal papilla,marginal type(91.67%)>parallel type(55.56%)>surrounding type(50.00%),the difference between the groups was statistically significant(P<0.05);however,the comparison of the success rate of stone retrieval between the PAD group(99.00%)and the non-PAD group(98.00%)was not statistically significant(χ^(2)=0.000,P=1.000).Conclusion The parapapillary diverticulum of the duodenum has little effect on the effect of endoscopic retrograde cholangiopancreatography;as long as the medical staff masters the operation skills,this kind of lithotripsy is effective for patients with parapapillary diverticulum of duodenum and choledocholithiasis.The treatment has a certain degree of safety and effectiveness.
