摘要
目的探讨冠状动脉粥样硬化性心脏病(冠心病)患者循环中长链非编码RNA(lncRNA CHAST)的表达和临床意义。方法选取2018年7月至2019年7月该院心内科诊治的115例冠心病患者作为冠心病组,选取同期年龄及性别相匹配的65例健康人作为对照组,收集两组临床资料和循环血清标本,实时荧光定量PCR和ELISA检测研究对象循环血清lncRNA CHAST表达水平和丝氨酸蛋白酶抑制剂A3(serpina3)浓度。根据造影Gensini评分将冠心病组患者分为轻度、中度和重度病变3个亚组,比较各组循环血清lncRNA CHAST表达水平和serpina3浓度之间的差异,探讨冠心病患者Gensini评分、血清lncRNA CHAST表达水平和serpina3浓度的相关性。采用受试者工作特征(ROC)曲线分析冠心病循环血清lncRNA CHAST表达水平和serpina3浓度对冠心病的诊断价值。结果冠心病组循环血清lncRNA CHAST表达水平和serpina3浓度均高于对照组,且轻、中、重度病变组血清lncRNA CHAST表达水平和serpina3浓度比较,差异有统计学意义(P<0.05)。lncRNA CHAST与Gensini评分呈正相关(r=0.783),serpina3与Gensini评分呈正相关(r=0.726),lncRNA CHAST与serpina3也呈正相关(r=0.901)。当lncRNA CHAST最佳截断值为0.92时,曲线下面积(AUC)为0.801(95%CI:0.765~0.847),灵敏度为75.1%,特异度为87.9%;当serpina3最佳截断值为99.37μg/mL时,AUC为0.914(95%CI:0.87~0.946),灵敏度为87.1%,特异度为93.9%;两者联合使用时,诊断冠心病患者的AUC为0.946(95%CI:0.897~0.982),灵敏度为91.1%,特异度为97.9%。结论lncRNA CHAST可作为冠心病早期诊断和严重程度评估的标志物。
Objective To investigate the expression and clinical significance of circulating long noncoding RNA(lncRNA CHAST)in patients with coronary heart disease.Methods From July 2018 to July 2019 in hospital,115 patients with coronary heart disease were selected as the coronary heart disease group,while 65 healthy people matched with age and gender who performed routine physical examination during the same period were collected as the control group.Clinical data and circulating serum samples of two groups of patients was collected.Real-time fluorescent quantitative PCR and ELISA were used to detect the expression level of lncRNA CHAST and serpina3 concentration in the circulating serum of the subjects.According to the Gensini score of angiography,patients in the coronary heart disease group were divided into three subgroups of the mild,moderate and severe disease.The differences between the circulating serum lncRNA CHAST expression level and serpina3 concentration of the groups were compared.And the correlation between Gensini score and serum lncRNA CHAST expression level and serpina3 concentration in patients with coronary heart disease were analyzed.Receiver operating characteristic(ROC)curve was used to analyze the clinical diagnostic value of circulating serum lncRNA CHAST expression level and serpina3 concentration in the diagnosis of coronary heart disease.Results The circulating serum lncRNA CHAST expression level and serpina3 concentration in the coronary heart disease group were significantly higher than those in the control group,and there was a significantly difference in serum lncRNA CHAST expression level and serpina3 concentration in the mild,moderate and severe coronary heart disease groups(P<0.05).LncRNA CHAST was positively correlated with Gensini score(r=0.783),serpina3 was positively correlated with Gensini score(r=0.726),and lncRNA CHAST was positively correlated with serpina3(r=0.901).When the cut-off value of lncRNA CHAST in serum of coronary heart disease was 0.92,area under the curve(AUC)was 0.801(95%CI:0.765-0.847),the sensitivity was 75.1%,the specificity was 87.9%.When the cut-off value of serpina3 concentration in serum of coronary heart disease was 99.37μg/mL,AUC was 0.914(95%CI:0.876-0.946),the sensitivity was 87.1%,the specificity was 93.9%.The combination of the two,AUC for the diagnosis of coronary heart disease was 0.946(95%CI:0.897-0.982),the sensitivity was 91.1%,the specificity was 97.9%.Conclusion LncRNA CHAST is biomarkers for early diagnosis and evaluation of severity in coronary heart disease.
