摘要
目的:探讨IL-18、IL-10在乳腺癌组织中的表达情况及其与淋巴结转移的相关性。方法:选取2017年11月-2019年6月山东大学附属济南市中心医院经手术切除并经病理确诊的104例乳腺癌组织及31例乳腺纤维腺瘤组织标本,应用免疫组织化学方法检测IL-18和IL-10的表达情况,比较乳腺癌组织及乳腺纤维腺瘤组织中IL-18和IL-10的表达情况;分析IL-18和IL-10表达的相关性;比较有无淋巴结转移的乳腺癌组织中IL-18和IL-10表达的表达情况;比较不同其他临床病理特征的乳腺癌组织中IL-18和IL-10表达情况。结果:乳腺癌组织的多数肿瘤细胞中IL-18表达阳性,其染色部位位于细胞膜及胞浆,呈弥漫性分布。乳腺癌组织中IL-18阳性表达率为75.0%(78/104),高于乳腺纤维腺瘤组织的19.4%(6/31),差异有统计学意义(P<0.001)。乳腺癌组织中多数细胞IL-10呈阳性表达,主要染色位于肿瘤细胞的胞浆。乳腺癌组织IL-10阳性表达率为78.8%(82/104)高于乳腺纤维腺瘤组织的22.6%(7/31)(P<0.001)。乳腺癌组织中IL-18与IL-10的表达呈正相关(r=0.533,P<0.05)。有淋巴结转移的乳腺癌组织中IL-18、IL-10均呈高表达;IL-18的阳性表达率为82.8%(53/64),IL-10的阳性表达率为85.9%(55/64),均高于无淋巴结转移的62.5%(25/40)、67.5%(27/40),差异均有统计学意义(P<0.05)。不同年龄、肿物最大直径、组织学类型、分化程度、临床分期、ER、PR和HER2/neu状态的乳腺癌组织中IL-18、IL-10的表达情况比较,差异均无统计学意义(P>0.05)。结论:与乳腺纤维腺瘤组织比较,IL-18、IL-10在乳腺癌组织中高表达,且两者的表达呈正相关,均与淋巴结转移具有一定相关性,IL-18与IL-10可能通过相互作用促进乳腺癌进展,两者可作为乳腺癌患者的诊断指标及不良预后标志物。
Objective:To investigate the expression of IL-18 and IL-10 in breast cancer tissues and their correlation with lymph node metastasis.Method:From November 2017 to June 2019 in Jinan Central Hospital affiliated to Shandong University,104 cases of breast cancer tissues and 31 cases of breast fibroadenoma tissue samples that were surgically resected and pathologically confirmed were selected to detect the expression of IL-18and IL-10 by immunohistochemistry.The expressions of IL-18 and IL-10 in breast cancer tissue and breast fibroadenoma tissue were compared.The correlation of IL-18 and IL-10 expression was analyzed.The expressions of IL-18 and IL-10 in breast cancer tissues with or without lymph node metastasis were compared.The expressions of IL-18 and IL-10 in breast cancer tissues with different clinicopathologic features were compared.Result:IL-18was positively expressed in most tumor cells of breast cancer tissue,and its staining sites were located in the cell membrane and cytoplasm,presenting a diffuse distribution.The positive expression rate of IL-18 in breast cancer tissue was 75.0% (78/104),which was higher than 19.4% (6/31) in breast fibroadenoma tissue,and the difference was statistically significant (P<0.001).IL-10 was positively expressed in most cells of breast cancer tissue,and mainly stained in the cytoplasm of tumor cells.The positive expression rate of IL-10 in breast cancer tissues was 78.8% (82/104),which was higher than 22.6% (7/31) in breast fibroadenoma tissues (P<0.001).There was a positive correlation between IL-18 and IL-10 expression in breast cancer tissue (r=0.533,P<0.05).IL-18 and IL-10 were highly expressed in the breast cancer tissues with lymph node metastasis;the positive expression rate of IL-18 was 82.8% (53/64),and that of IL-10 was 85.9% (55/64),both of which were higher than 62.5% (25/40) and 67.5% (27/40) in breast cancer tissues without lymph node metastasis,with statistical significance (P<0.05).There were no statistical significance in the expressions of IL-18 and IL-10 in breast cancer tissues with different ages,maximum tumor diameter,histological type,degree of differentiation,clinical stage,ER,PR and HER2/neu status (P>0.05).Conclusion:Compared with breast fibroadenoma tissue,IL-18 and IL-10 are highly expressed in breast cancer tissue,and their expressions are positively correlated and correlated with lymph node metastasis.IL-18 and IL-10 may promote the progression of breast cancer through interaction,and both can be used as diagnostic indicators and poor prognostic markers for breast cancer patients.
