摘要
文章以两种慢性疾病——精神分裂症和慢性阻塞性肺疾病为例,通过经济学综述,对药物经济学评价中依从性和停药率常见处理方法进行归纳总结;再构建马尔科夫模型对慢性阻塞性肺疾病进行经济学评价,设定不同停药率情境,分析停药率对于研究结果的影响。实例研究结果显示:当年A药和B药的停药率取相同数值时,结果较为稳定;A药和B药分别取不同的停药率时,结果影响较大,且当B药停药率低于A药时,结果会发生反转。药物经济学评价中可依据使用的模型类别灵活选择纳入依从性或停药率的方式,推荐将药品依从性或者停药率纳入到不确定性分析中进行进一步探究。
Schizophrenia and chronic obstructive pulmonary disease(COPD) were chosen as typical chronic diseases.A review of economic evaluation was conducted to summarize the methods.Also,a case study was used where a Markov model for COPD with scenario analysis for different discontinuation rates was developed to compare the impact on the results.In the case study,when discontinuation rates of drug A and drug B were the same,the results were totally stable.However,when discontinuation rates were different for drug A and drug B,certain changes could be observed especially for relatively low discontinuation of drug B which could reverse the results.Compliance and discontinuation rate should be included based on the model types.It was recommended to include compliance and discontinuation in the uncertainty analysis to further explore the impact on the pharmacoeconomic results.
