摘要
通过深入研究脑卒中患者的康复特点及康复需求,借鉴以往文献研究成果,遵照国家下发的家庭医生制度指导意见,提出由三甲医院医疗团队与社区卫生服务中心医疗团队共同组建家庭医生团队,对脑卒中患者实行签约上门服务的康复模式。这样既减轻了患者的经济负担和家庭照顾负担,又提高了社区康复的技能,真正实现医院式康复与居家式康复相结合,从而促进脑卒中患者的功能恢复。
By an in-depth study of the rehabilitation characteristics and rehabilitation needs of patients,drawing on previous literature research results and following the guidance of the family doctor system issued by the state,the medical team of a third-class hospital and community medical team are proposed to establish a family doctor team and implement a rehabilitation mode of contracted on-site service for stroke patients.It not only reduces the economic burden and family care burden of patients,but also improves the skill of community rehabilitation.It really realizes the combination of hospital and home rehabilitation,so as to promote the recovery of the body function of stroke patients.
