摘要
目的探讨无创呼吸机辅助通气下枸橼酸咖啡因对新生儿呼吸暂停的治疗效果。方法抽取2018年2月至2020年1月平顶山市第一人民医院收治的新生儿呼吸暂停患儿88例,根据随机数字表法将所有患儿分为对照组与观察组,每组44例。对照组在无创呼吸机辅助通气下加用氨茶碱治疗,观察组在无创呼吸机辅助通气下加用枸橼酸咖啡因治疗。记录两组患儿无创辅助通气时间、住院时间、氧疗时间、临床疗效、肺功能、不良反应及复发率。结果观察组氧疗时间、无创辅助通气时间、住院时间短于对照组(P<0.05);观察组每分钟通气量、潮气量、达峰时间比、达峰容积比高于对照组(P<0.05);观察组呼吸频率高于对照组,但P>0.05;观察组心动过速、高血糖、喂养不耐受等不良反应发生率与复发率低于对照组(P<0.05);对照组血压下降发生率高于观察组,组间比较P>0.05。结论对新生儿呼吸暂停行无创呼吸机辅助通气下枸橼酸咖啡因治疗,能有效缩短治疗时间,改善肺功能,提高临床疗效,降低复发率和不良反应发生率,对患儿影响更小,安全性较高。
Objective To investigate the curative effects of caffeine citrate assisted by noninvasive ventilation(NIV)in treatment of neonatal apnea.Methods A total of 88 children with neonatal apnea in the Pingdingshan First People’s Hospital from February 2018 to January 2020 were selected,and all the children were divided into control group and observation group according to the random number table method,with 44 cases in each group.Control group was given NIV assisted aminophylline,while observation group was treated with NIV assisted caffeine citrate.The NIV time,hospital stays,oxygen therapy time,clinical efficacy,pulmonary function,adverse reactions and recurrence rate in the two groups were recorded.Results The time of oxygen therapy,noninvasive ventilation and hospital stay in the observation group were shorter than those in the control group(P<0.05);the minute ventilation volume,tidal volume,peak time ratio and peak volume ratio of the observation group were higher than those of the control group(P<0.05);the respiratory rate in observation group was higher than that in control group,but P>0.05;the incidence of tachycardia,hyperglycemia,feeding intolerance and the recurrence rate in observation group were lower than those in control group(P<0.05);the incidence of decreased blood pressure in control group was higher than that in observation group,but P>0.05.Conclusions In the treatment of neonatal apnea,caffeine citrate assisted by noninvasive ventilation can effectively shorten the treatment time,improve pulmonary function,increase clinical efficacy,decrease the recurrence rate and the incidence of adverse reactions,which has less adverse influence and higher safety.
