摘要
目的探讨急诊留观患者疾病不确定感、应对适应能力与心理应激反应间的关系,为降低患者心理应激水平提供参考。方法采用一般资料调查表、中文版Mishel疾病不确定感量表、中文版简易罗伊应对适应能力量表及心理应激反应问卷对大连医科大学附属第二医院260例急诊留观患者进行问卷调查,并对结果进行分析。结果应对适应能力在急诊留观患者疾病不确定感与心理应激反应间起部分中介作用,中介效应占总效应的32.23%;应对适应能力可通过情绪及行为反应路径减少疾病不确定感对心理应激反应的正性增强作用。结论疾病不确定感可直接或通过应对适应能力间接影响急诊留观患者的心理应激反应,可通过降低疾病不确定感、提高应对适应能力来减少心理应激反应对患者心身功能的影响。
Objective To investigate the relationship between uncertainty in illness,coping with adaptive ability and psychological stress response among patients in Emergency Observation Room,and to provide reference for reducing the psychological stress level of patients.Methods Questionnaire survey was conducted on 260 patients in Emergency Observation Room in a third-grade hospital in Dalian using the general data questionnaire,the Chinese version of the Mishel’Uncertainty in Illness Scale,the Chinese version of the Coping Adaptation Processing Scale and the Stress Response Questionnaire.Results Coping with adaptive ability played a part in mediating the relationship between uncertainty in illness and psychological stress among patients in Emergency Observation Room.The mediating effect accounted for 32.23%of the total effect.Coping with adaptive ability can reduce the positive reinforcement effect of uncertainty in illness on psychological stress response through emotional and behavioral response pathways.Conclusions Uncertainty in illness can directly or indirectly affect the psychological stress response of by responding to coping with adaptive ability.It can reduce the influence of psychological stress response on patients'psychosomatic function by reducing the uncertainty in illness and improving the ability to cope with adaptation.
