经颅多普勒超声监测联合尿激酶静脉溶栓治疗急性脑梗死临床观察 被引量:2

Clinical observation of transcranial Doppler ultrasound monitoring combined with intravenous thrombolysis with urokinase in the treatment of acute cerebral infarction

摘要 目的观察经颅多普勒超声(TCD)监测联合尿激酶静脉溶栓治疗急性脑梗死的临床效果。方法40例急性脑梗死患者作为研究对象,均实施尿激酶溶栓治疗,在溶栓治疗的过程中使用经颅多普勒超声进行持续监测,评估比较患者溶栓前后大脑中动脉收缩期峰血流速度(MCA-Vp)、大脑中动脉平均血流速度(MCA-Vm)、动脉搏动指数(PI)、美国国立卫生研究院卒中量表(NIHSS)评分。结果患者溶栓前的MCA-Vp为(19.25±6.64)cm/s、MCA-Vm为(10.31±15.34)cm/s、PI为(0.87±0.07),溶栓后即刻分别为(87.05±14.25)cm/s、(63.21±6.12)cm/s、(0.60±0.11);患者溶栓后即刻的MCA-Vp、MCA-Vm高于溶栓前,PI低于溶栓前,差异具有统计学意义(P<0.05)。患者溶栓后NIHSS评分(7.04±2.10)分明显低于溶栓前的(15.20±3.01)分,差异具有统计学意义(P<0.05)。结论急性脑梗死患者在静脉溶栓过程中通过经颅多普勒超声持续监测,可以有效对患者的血流动力学情况进行监测评估,评价患者的血管再通状况,对监测患者的病症及预后发挥了重要作用,也可以为临床的治疗提供指导。 Objective To observe the clinical effect of transcranial Doppler(TCD)ultrasound monitoring combined with intravenous thrombolysis with urokinase in the treatment of acute cerebral infarction.Methods 40 patients with acute cerebral infarction were studied and all were treated with urokinase thrombolysis,which was continuously monitored using transcranial Doppler ultrasound during thrombolysis.The peak systolic velocity in the middle cerebral artery(MCA-Vp),mean velocity in the middle cerebral artery(MCA-Vm),arterial pulsatility index(PI),and the National Institutes of Health stroke scale(NIHSS)score were evaluated and compared before and after thrombolysis.Results MCA-Vp,MCA-Vm and PI of patients before thrombolysis were(19.25±6.64),(10.31±15.34)and(0.87±0.07)cm/s and immediately after thrombolysis were(87.05±14.25),(63.21±6.12)and(0.60±0.11)cm/s,respectively.MCA-Vp and MCA-Vm in patients immediately after thrombolysis were higher than before thrombolysis,and the levels of PI were lower than that thrombolysis.All the differences were statistically significant(P<0.05).NIHSS score(7.04±2.10)points of patients after thrombolysis was significantly lower than(15.20±3.01)points before thrombolysis,and the difference was statistically significant(P<0.05).Conclusion Continuous monitoring by transcranial Doppler ultrasound during intravenous thrombolysis in patients with acute cerebral infarction can effectively monitor and assess the hemodynamic condition of patients and evaluate their revascularization status,which plays an important role in monitoring patients’disease and prognosis and can also provide guidance for clinical treatment.
