摘要
目的观察含奥沙利铂化疗方案用于治疗原发性肝癌的临床疗效及对患者免疫功能及肿瘤标志物的影响。方法选取100例原发性肝癌患者,通过随机数字表法分为对照组和观察组,各50例。对照组采用常规化疗方案治疗,观察组在对照组基础上加用奥沙利铂进行治疗,4周为1个周期,2组患者均治疗3个周期。观察并比较2组患者治疗前后免疫功能、肿瘤标志物、肝功能、疗效、生活质量和不良反应发生率。结果治疗后,观察组T细胞标志物CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)和自然杀伤细胞(NK)水平均高于对照组(P<0.05);观察组血清癌胚抗原(CEA)、甲胎蛋白(AFP)、糖类抗原199(CA199)和糖类抗原(CA125)水平低于对照组(P<0.05);观察组谷氨酸-丙酮酸转氨酶(ALT)、总胆红素(TBIL)和γ-谷氨酰转肽酶(GGT)水平均低于对照组,观察组白蛋白(ALB)水平高于对照组(P<0.05)。观察组治疗总有效率、生活质量总改善率高于对照组(P<0.05),不良反应发生率低于对照组(P<0.05)。结论采用含奥沙利铂化疗方案进行治疗,可以提高原发性肝癌患者免疫能力,提高治疗有效率,具有较好的应用效果。
Objective To observe the clinical efficacy,immune function and tumor markers in patients with primary liver cancer treated with oxaliplatin.Methods 100 patients with primary liver cancer were selected and randomly divided into the control group and the observation group,50 cases in each group.The control group was treated with conventional chemotherapy,and the observation group was treated with oxaliplatin on the basis of the control group,4 weeks as a cycle,both groups were treated for 3 cycles.The immune function,tumor markers,liver function,curative effect,quality of life and incidence of adverse reactions were observed and compared between the 2 groups before and after treatment.Results After treatment,the levels of T cell markers CD3^(+),CD4^(+),CD4^(+)/CD8^(+)and NK in the observation group were higher(P<0.05);The serum levels of CEA,AFP,CA199 and CA125 in the observation group were lower(P<0.05).The levels of glutamic pyruvate transaminase(ALT),total bilirubin(TBIL)and gamma glutamyl transpeptidase(GGT)in the observation group were lower,and the albumin(ALB)level in the observation group was higher(P<0.05);The total effective rate and improvement rate of the observation group were higher(P<0.05);The incidence of adverse reactions in the observation group was lower.Conclusion The use of oxaliplatin containing chemotherapy can improve the immune capacity of patients with primary liver cancer,improve the treatment efficiency,and has good application effect.
