摘要
目的探讨帕金森病伴日间嗜睡(PD-EDS)患者静息态下脑区低频振幅(ALFF)和功能连接(FC)的改变。方法选取年龄、性别、病程、受教育程度等分别匹配的PD-EDS组患者14例及帕金森病不伴日间嗜睡患者(PD-NEDS组)18例,两组均进行脑静息态功能磁共振扫描,计算ALFF值,以ALFF的差异脑区作为种子点,计算其与全脑的FC值。结果与PD-NEDS组相比,PD-EDS组在右侧额下回、左侧嗅皮质、左侧额中回等脑区ALFF值减低。FC分析显示,在PD-EDS组中,ALFF值有差异的3个脑区与胼胝体、双侧额中回、双侧扣带回、右侧小脑后叶、右侧楔叶、右侧额内侧回、左侧枕中回、左侧额叶直回等脑区的FC下降。结论PD-EDS组与PD-NEDS组存在ALFF的差异脑区,且存在不同的FC改变,有助于进一步探讨PD伴有EDS的神经机制。
Objective To investigate the changes of the amplitude of low-frequency fluctuation(ALFF)and the functional connectivity(FC)of brain in resting state of patients with Parkinson's disease and excessive daytime sleepiness(PDEDS).Methods Fourteen patients in PD-EDS group and eighteen patients in PD-NEDS group(Parkinson's disease without excessive daytime sleepiness)who were matched in age,gender,course of disease and education level were selected.Both groups were scanned with brain resting-state functional magnetic resonance(fMRI)to calculate the ALFF value.And then,the brain regions with different ALFF values were selected as the seed to calculate the FC of the whole brain.Results Compared with the PD-NEDS group,the ALFF of the right inferior frontal gyrus,left olfactory and left middle frontal gyrus decreased in the PD-EDS group.FC analysis showed that in the PD-EDS group,the functional connections between the three brain areas with different ALFF values and the corpus callosum,bilateral middle frontal gyrus,bilateral cingulate gyrus,right cerebellum posterior lobe,right cuneus,right medial frontal gyrus,left middle occipital gyrus and left rectal gyrus decreased.Conclusion The PD-EDS group and PD-NEDS group have different brain areas with amplitude of low frequency fluctuation and different functional connectivity changes,which is helpful to further explore the neural mechanism of PD with EDS.
