摘要
目的评价香藿喷雾剂联合基础康复疗法治疗新型冠状病毒肺炎恢复期余毒未清证的临床疗效和安全性。方法将120例新型冠状病毒肺炎恢复期余毒未清证患者随机分为治疗组和对照组各60例,在基础康复疗法的基础上治疗组予以香藿喷雾剂喷喉,每日早、中、晚各1次,每次3揿,症状明显时可适当加量,但每日总剂量不超过18揿;对照组口服乙酰半胱氨酸片,每次0.2g,每日3次。两组疗程均为7天,必要时可酌情延长,但不超过14天。观察两组患者治疗前后中医证候积分(主症包括咳嗽、痰黏难咯、胸闷,次症包括乏力、失眠、汗出、纳差、肌肉酸痛、恶心、呕吐、腹泻、鼻塞、流涕、咽痛)、新型冠状病毒核酸复阳率,治疗后判定中医证候疗效,并记录两组严重不良事件发生情况。结果最终两组各59例纳入统计分析,治疗组临床痊愈5例,显效36例,有效17例,无效1例,愈显率为69.49%,总有效率98.30%;对照组分别为1例、13例、37例、8例、23.73%及86.44%,治疗组愈显率和总有效率均高于对照组(P<0.01)。治疗组治疗后各症状积分均较治疗前明显降低(P<0.05或P<0.01);对照组治疗后除鼻塞、恶心、呕吐改善不明显外(P>0.05),其他症状积分均较治疗前降低(P<0.05或P<0.01);且治疗后治疗组咳嗽、胸闷、乏力、汗出、肌肉酸痛、鼻塞、咽痛等症状积分降低均较对照组更显著(P<0.05或P<0.01)。两组患者治疗后中医证候总积分均较治疗前降低(P<0.01),且治疗组改善显著优于对照组(P<0.01)。两组患者研究期间均未出现新型冠状病毒核酸检测复阳情况,且均未发生严重不良事件。结论香藿喷雾剂联合基础康复疗法治疗新型冠状病毒肺炎恢复期余毒未清证临床疗效较好,能明显改善恢复期症状,并且安全性良好。
Objective To evaluate the clinical effectiveness and safety of Xiang Huo Spray(香藿喷雾剂)in the treatment of coronavirus disease 2019(COVID-19)with syndrome of residual toxin during the convalescence period.Methods Totally 120 patients with COVID-19 of residual toxin syndrome in the convalescence period were randomly divided into a treatment group and a control group with 60 cases in each group.Based on the basic rehabilitation therapy,patients in the treatment group received throat spray(Xiang Huo Spray)once in the morning,once at noon and again in the evening,with three presses each time;the dose can be increased to no more than 18 presses each day when the symptoms were obvious;patients in the control group were given oral acetylcysteine tablets,0.2 g each time,three times daily;the treatment course lasted for 7 days for both groups,which could be extended to no more than 14 days as appropriate if necessary.The improvement of traditional Chinese medicine(TCM)symptoms(primary symptoms:cough,sticky sputum hard to expectorate,chest oppression;secondary symptoms:fatigue,insomnia,sweating,poor appetite,nausea and vomiting,diarrhea,stuffy nose,runny nose,sore throat),re-positive rate of nucleic acid test result,and severe adverse events of the two groups were observed.Results Fifty-nine patients in each group were finally analyzed.In the treatment group,5 cases were cured;36 cases were markedly effective;17 cases were effective;and one case was ineffective;the cured and markedly effective rate and the total effective rate were 69.49% and 98.31%,respectively;the corresponding numbers and rates in the control group were successively 1,13,37,8,23.73% and 86.44%;the cured and markedly effective rate and the total effective rate in the treatment group were higher than those in the control group(P<0.01).After treatment,the TCM symptoms scores the treatment group were significantly reduced than those before treatment(P<0.05 or P<0.01);the symptoms scores except for nasal congestion and vomiting and nausea in the control group were significantly lower than those before treatment(P<0.05 or P<0.01);more decreases were seen in the treatment group than in the control group regarding the scores of cough,chest oppression,fatigue,sweating,muscle soreness,stuffy nose and sore throat(P<0.05 or P<0.01);the total score of TCM symptoms in both groups were lower than those before treatment(P<0.01),and there was more decrease in the treatment group(P<0.01).There was no re-positive case of nucleic acid test,as well as no serious adverse events in both groups.Conclusion Xiang Huo Spray combined with basic rehabilitation therapy has certain clinical effects in treating COVID-19 with residual toxin syndrome in the convalescence period by significantly improving the symptoms,and was safe.
