摘要
目的探讨单侧颈内动脉或大脑中动脉狭窄患者颅脑定量磁敏感成像序列(quantitative susceptibility mapping,QSM)与脑血容量(cerebral blood volume,CBV)变化的相关性。方法选取26例单侧颈内动脉或大脑中动脉狭窄患者,均在3.0 T Siemens Magnetom Prisma扫描仪上使用64通道相控阵头部线圈进行定量磁敏感成像、动态磁敏感灌注加权成像及头颈部高分辨率磁共振血管壁成像检查(HR-MRI),获取了相应的QSM、CBV及HR-MRI图像。通过选择病变侧及对侧相应部位皮层下感兴趣区测量后进行Pearson检验,研究病变侧与对侧皮层下脑组织QSM差值(ΔQSM)与CBV差值(ΔCBV)的关系。结果26例单侧头颈部动脉狭窄患者,其中12例颈内动脉狭窄、12例大脑中动脉狭窄、2例同时存在同侧颈内动脉及大脑中动脉狭窄;病变侧与对侧皮层下脑组织ΔQSM与ΔCBV存在显著正相关关系(r=0.756,P<0.01)。结论QSM有潜力成为评价单侧头颈部血管病变患者脑血容量的无创定量影像学技术。
Objective To explore the relationship between quantitative susceptibility mapping(QSM)and pathological cerebral blood volume(CBV)in patients with unilateral carotid artery or middle cerebral artery stenosis.Methods Totally,26 patients with unilateral carotid artery or middle cerebral artery stenosis were recruited in this study.Scanning was performed on the 3.0 T Siemens Magnetom Prisma scanner.MR images of all patients were acquired,including QSM,relative CBV and High resolution magnetic resonance imaging of carotid and cerebral arteries.By selecting a subcortical region of interest(ROI)of the affected and contralateral brain tissue followed by Pearson test,the correlation betweenΔQSM andΔCBV was evaluated.Results In 26 patients with unilateral carotid artery stenosis,there were 12 patients with unilateral middle cerebral artery stenosis and 2 patients with both unilateral carotid artery and middle cerebral artery stenosis.We found a strong positive correlation withΔQSM andΔCBV(r=0.756,P<0.01).Conclusion QSM holds the potential for assessment of pathological cerebral blood volume in patients with unilateral carotid artery and middle cerebral artery stenosis non-invasively.
