低频脉冲电刺激联合肢体康复锻炼对脑卒中偏瘫患者PSQI总分、凝血功能和肢体运动功能的影响 被引量:46

摘要 目的分析低频脉冲电刺激联合肢体康复锻炼对脑卒中偏瘫患者匹兹堡睡眠质量指数(PSQI)总分、凝血功能和肢体运动功能的影响。方法回顾性选取2018年8月至2020年8月合肥市第二人民医院收治的60例脑卒中偏瘫患者作为研究对象,按治疗方法的不同分为2组,对照组30例采用基础治疗+肢体康复锻炼,观察组30例采用基础治疗+低频脉冲电刺激疗法+肢体康复锻炼。观察并记录2组治疗前、治疗1、2个月后PSQI总分及凝血功能指标[活化部分凝血活酶时间(APTT)、纤维蛋白原(FIB)、凝血酶原时间(PT)、D-二聚体(D-D)]水平及肢体运动功能[上肢Fugl-Meyer运动功能(FMA)评分和动作研究量表(ARAT)评分]。结果治疗前,2组PSQI总分、凝血功能指标、肢体运动功能比较,差异均无统计学意义(P>0.05)。治疗1、2个月后,观察组PSQI总分为(10.91±1.25)、(5.14±0.75)分,低于对照组[(11.47±1.04)、(10.85±0.96)分],差异均有统计意义(P<0.05)。治疗2个月后,观察组APTT、PT、D-D水平为(36.57±4.25)s、(15.96±1.51)s、(1.07±0.05)mg/L,高于对照组[(31.15±4.38)s、(13.29±1.58)s、(0.53±0.07)mg/L],FIB水平(2.06±0.47)g/L低于对照组(2.40±0.45)g/L,差异均有统计意义(P<0.05)。治疗1、2个月后,观察组FMA、ARAT为(35.04±2.79)、(42.93±5.44)分和(26.41±2.11)、(35.87±4.75)分,均高于对照组[(32.31±2.73)、(36.69±4.23)分和(20.13±1.85)、(27.13±3.85)分],差异均有统计意义(P<0.05)。结论在同等基础治疗下,低频脉冲电刺激联合肢体康复锻炼对改善脑卒中偏瘫患者睡眠质量、凝血功能和肢体运动功能更有优势,值得临床予以推广应用。 Objective To analyze the effect of low-frequency pulse electrical stimulation combined with limb rehabilitation exercise on the total score of Pittsburgh Sleep Quality Index(PSQI),coagulation function and limb motor function in patients with hemiplegia after stroke.Methods Sixty patients with stroke and hemiplegia admitted to Hefei Second People's Hospital from August 2018 to August 2020 were retrospectively selected as the research objects.They were divided into 2 groups according to different treatment methods.Thirty patients in the control group were treated with basic treatment+physical rehabilitation exercises,30 cases in the observation group were treated with basic treatment+low-frequency pulse electrical stimulation therapy+limb rehabilitation exercise.The total PSQI scores and coagulation function[activated partial thromboplastin time(APTT),fibrinogen(FIB),prothrombin time(PT),D-dimer(DD)]expression level and limb motor function[Upper Limb Fugl-Meyer Motor Function(FMA)Score and Movement Research Scale(ARAT)Score]were observed and recorded before treatment,1 month after treatment,and 2 months after treatment in the two groups.Results Before treatment,there was no statistically significant difference in the total PSQI score,coagulation function index,and limb motor function between the two groups(P>0.05);after 1 and 2 months of treatment,the total PSQI scores in the observation group were(10.91±1.25)and(5.14±0.75)points,which were lower than those in the control group[(11.47±1.04),(10.85±0.96)points],and the differences were statistically significant(P<0.05).After 2 months of treatment,the levels of APTT,PT,and DD in the observation group were(36.57±4.25)s,(15.96±1.51)s,and(1.07±0.05)mg/L,which were higher than those in the control group[(31.15±4.38)s,(13.29±1.58)s,(0.53±0.07)mg/L],and the level of FIB was(2.06±0.47)g/L,which was lower than that in the control group[(2.40±0.45)g/L],the differences were statistically significant(P<0.05).After 1 and 2 months of treatment,the FMA and ARAT scores in the observation group were(35.04±2.79),(42.93±5.44)points and(26.41±2.11),(35.87±4.75)points,which were higher than those in the control group[(32.31±2.73),(36.69±4.23)points and(20.13±1.85),(27.13±3.85)points],the differences were statistically significant(P<0.05).Conclusion Under the same basic treatment,low-frequency pulse electrical stimulation combined with limb rehabilitation exercise has more advantages in improving the sleep quality,blood coagulation function and limb motor function of patients with stroke and hemiplegia,and it is worthy of clinical application.
