摘要
目的:对2019年山东省中医医疗机构老年人群治疗费用的主要特征进行分析核算,为减轻老年人疾病经济负担、促进中医药事业发展提供依据。方法:基于卫生服务核算体系2011方法和理念,从治疗费用总量、年龄分布、机构流向、疾病分布以及筹资来源分布方面,对山东省中医医疗机构65岁及以上老年人的治疗费用进行核算。结果:2019年山东省中医医疗机构65岁及以上老年人的治疗费用为1017063.10万元,占治疗总费用的36.05%;从ICD-10分类来看,居于前五位的疾病类型分别为循环系统疾病、呼吸系统疾病、肿瘤、损伤、中毒和外因某些其他后果以及肌肉骨骼肌和结缔组织疾病;治疗费用主要流向医院;公共筹资方案总额为686295.87万元,是治疗费用的主要筹资来源,占比67.47%。结论:加强对老年人重点疾病及经济负担的关注;提高基层医疗机构的服务能力,合理引导就医流向;优化筹资结构,充分发挥中医诊疗优势。
Objective:To analyze and calculate the main characteristics of the treatment cost for the elderly in TCM hospitals in Shandong Province in 2019,so as to provide references for reducing the economic burden of diseases of the elderly and promoting the development of TCM.Methods:Based on SHA 2011,the treatment cost of the elderly aged 65 and above in TCM hospitals in Shandong Province was calculated from the total amount of treatment cost,age distribution,institution flow,disease distribution and financing distribution.Results:In 2019,the treatment expense of the elderly aged 65 and above in TCM medical institutions in Shandong Province was 10170.6310 million yuan,accounting for 36.05%of the total treatment expense.According to the ICD-10 classification,the top 5 diseases were circulatory diseases,respiratory diseases,tumors,injuries,poisonings and some other external causes,and musculoskeletal muscle and connective tissue diseases.The cost of treatment flows mainly to hospitals.The public financing scheme was the main source of financing for treatment costs,accounting for 6862.9587 million yuan,accounting for 67.47%.Conclusion:It was needed to strengthen the attention to the important diseases and economic burden of the elderly;improve the service capacity of primary medical institutions,guide the flow of medical treatment reasonably;optimize the financing structure and give full play to the advantages of TCM diagnosis and treatment.
