摘要
目的观察祛风散寒利湿方联合甲氨蝶呤在急性期寒湿痹阻证类风湿性关节炎(Rheumatoid arthritis,RA)的临床疗效。方法选取2018年10月—2020年6月期间空军军医大学第一附属医院门诊部收治的102例RA患者,按照随机数字表法分为对照组与治疗组,每组51例。对照组给予甲氨蝶呤治疗,治疗组给予甲氨蝶呤联合祛风散寒利湿方治疗,两组患者均治疗8周。统计比较两组患者临床疗效、治疗安全性、中医证候积分、疾病活动度(DAS28)评分、生活质量(QLICD-RA)评分变化情况。结果两组患者治疗后,治疗组总有效率为92.16%,对照组为78.43%,两组比较,差异有统计学意义(P<0.05)。治疗后两组患者中医证候积分、DAS28评分均较治疗前降低,且治疗组均较对照组低,差异有统计学意义(P<0.05)。治疗后两组患者QLICD-RA各维度评分均较治疗前增高,且治疗组较对照组高,差异有统计学意义(P<0.05)。两组患者不良反应发生率比较,差异无统计学意义(P>0.05)。结论祛风散寒利湿方联合甲氨蝶呤治疗急性期寒湿痹阻证RA安全、有效,有利于降低疾病活动度,改善患者生活质量。
Objective To explore the therapeutic effect of Qufeng Sanhan Lishi Decoction combined with methotrexate in patients with rheumatoid arthritis(RA)in acute cold-dampness syndrome.Methods 102 RA patients in our hospital were divided into control group and treatment group according to random number table method,with 51 cases in each group.The control group was given methotrexate treatment,and the treatment group was given methotrexate+Qufeng Sanhan Lishi Fang treatment,both groups were treated for 8 weeks.The clinical efficacy and treatment safety of the two groups were statistically compared,and the changes of TCM syndrome score,disease activity score(DAS28),quality of life score(QLICD-RA)before and after treatment were compared between the two groups.Results After the two groups of patients were treated,the total effective rate was 92.16%in the treatment group and 78.43%in the control group.The difference between the two groups was statistically significant(P<0.05).After treatment,the TCM syndrome scores and DAS28 scores of the two groups were lower than before treatment,and the treatment group was lower than the control group,the difference was statistically significant(P<0.05).After treatment,the QLICD-RA scores of the two groups of patients were higher than before treatment,and the treatment group was higher than the control group,the difference was statistically significant(P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups of patients(P>0.05).Conclusion Qufeng Sanhan Lishi Decoction combined with methotrexate is safe and effective in the treatment of acute cold-dampness-blocking RA,which is beneficial to reduce disease activity and improve the quality of life of patients.
