摘要
目的:比较胸腔镜下肺段切除术与肺叶切除术治疗肺部小结节的效果。方法:2018年1月-2020年10月收治肺部小结节患者60例,随机分为两组,各30例。对照组实施胸腔镜下肺段切除术,试验组实施胸腔镜下肺叶切除术,比较两组患者临床指标水平及肺功能变化。结果:试验组手术时间、术后住院时间及术中出血量均低于对照组,差异有统计学意义(P<0.05);试验组治疗后血清铁蛋白、NSE、CEA、CA125、CYFRA21-1等指标水平均明显低于对照组,差异有统计学意义(P<0.05);两组治疗后肺功能指标水平均有显著下降,但试验组治疗后用力肺活量(FVC)及第1秒用力呼吸容积(FEV1)水平均明显高于对照组,差异有统计学意义(P<0.05)。结论:采用胸腔镜下肺段切除术治疗肺部小结节的疗效优于胸腔镜下肺叶切除术,对患者肺功能的恢复具有积极意义。
Objective:To compare the effects of thoracoscopic segmental pneumonectomy and lobectomy in the treatment of small pulmonary nodules.Methods:From January 2018 to October 2020,60 patients with small pulmonary nodules were selected.They were randomly divided into two groups with 30 cases in each group.The control group underwent thoracoscopic segmental pneumonectomy.The experimental group underwent thoracoscopic lobectomy.We compared the changes of clinical indexes and pulmonary function between the two groups.Results:In the experimental group,the operation time,postoperative hospital stay and intraoperative bleeding were lower than those in the control group,the difference was statistically significant(P<0.05).After treatment,the serum ferritin,NSE,CEA,CAl25 and CYFRA21-1 in the experimental group were significantly lower than those in the control group,the difference was statistically significant(P<0.05).The levels of pulmonary function indexes in the two groups decreased significantly after treatment,but the levels of forced vital capacity(FVC)and forced respiratory volume in one second(FEV1)in the experimental group were significantly higher than those in the control group,the difference was statistically significant(P<0.05).Conclusion:The curative effect of thoracoscopic segmental pneumonectomy in the treatment of small pulmonary nodules is better than thoracoscopic lobectomy,which is of positive significance to the recovery of pulmonary function.
