摘要
目的:探讨急诊早期应用呼吸机抢救急性重症有机磷农药中毒致呼吸衰竭的临床效果。方法:选择2017年1月~2020年6月60例急性重症有机磷农药中毒致呼吸衰竭患者为对象,按照入院顺序将患者分成参照组(n=30)和研究组(n=30)。参照组采用常规方法治疗,研究组采用常规方法联合呼吸机抢救治疗。结果:治疗后研究组的血氧分压低于参照组,血二氧化碳分压与pH低于参照组,自主呼吸恢复和意识状态恢复时间短于参照组,抢救成功率高于参照组,P<0.05。研究组的不良反应发生率稍高于参照组,但差异无统计学意义,P>0.05。结论:急诊早期应用呼吸机抢救急性重症有机磷农药中毒致呼吸衰竭的成功率高,可显著改善患者的血气指标与机体酸碱度,促使患者尽快恢复自主呼吸与意识,且不增加患者发生不良反应的风险。
Objective:To investigate the clinical effect of ventilator in emergency treatment of respiratory failure caused by acute severe organophosphorus pesticide poisoning.Methods:From January 2017 to June 2020,60 patients with respiratory failure caused by acute severe organophosphorus pesticide poisoning were selected as subjects,and the patients were divided into the reference group(n=30)and the study group(n=30)according to the admission sequence.The reference group was treated with conventional methods,while the study group was treated with conventional methods combined with ventilator.Results:After treatment,the blood oxygen partial pressure,blood carbon dioxide partial pressure and pH of the study group were lower than those of the control group,the recovery time of spontaneous respiration and consciousness state was shorter than that of the control group,and the rescue success rate was higher than that of the control group,P<0.05.The incidence of adverse reactions in the study group was slightly higher than that in the control group,but the statistically significant difference was insignificant,P>0.05.Conclusion:Early application of ventilator in emergency rescue of respiratory failure caused by acute severe organophosphorus pesticide poisoning has a high success rate,can significantly improve the blood gas index and body pH,promote the recovery of spontaneous breathing and consciousness in patients as soon as possible,and does not increase the risk of adverse reactions in patients.
