摘要
目的探讨常规药物联合低分子量肝素钙治疗小儿过敏性紫癜肾炎(HSPN)的临床效果。方法选取2016年2月至2019年5月我院收治的80例HSPN患儿作为研究对象,按照随机数字法将其分为对照组(38例)和研究组(42例)。对照组给予常规药物治疗,研究组在对照组基础上联合低分子量肝素钙治疗。比较两组的临床疗效、临床症状消失时间、免疫功能指标、凝血功能指标、炎症因子指标、肾功能指标。结果研究组的治疗总有效率明显高于对照组(P<0.05)。研究组的紫癜消失时间、尿蛋白转阴时间、消化道症状消失时间、关节症状消失时间明显短于对照组(P<0.05)。治疗后,研究组的IgA、IgG、IL-17、TNF-α、ALB水平及24 h尿蛋白定量明显低于对照组,APTT、PT明显长于对照组(P<0.05)。结论常规药物联合低分子量肝素钙治疗小儿HSPN的效果显著,可缩短临床症状消失时间,改善免疫功能、凝血功能、肾功能,减轻炎症反应。
Objective To investigate the clinical effect of conventional drugs combined with low molecular weight heparin calcium in the treatment of Henoch-Schonlein purpura nephritis(HSPN)in children.Methods Eighty children with HSPN treated in our hospital from February 2016 to May 2019 were selected as the research objects.The children were divided into control group(38 cases)and study group(42 cases)according to random number method.The control group was treated with conventional drugs,and the study group was treated with low molecular weight heparin calcium on the basis of the control group.The clinical efficacy,disappearance time of clinical symptoms,the indexes of immune function,coagulation function,inflammatory factor and renal function were compared between the two groups.Results The total effective rate of treatment in the study group was significantly higher than that in the control group(P<0.05).The disappearance time of purpura,the negative conversion time of urinary protein,the disappearance time of gastrointestinal symptoms and joint symptoms in the study group were significantly shorter than those in the control group(P<0.05).After treatment,IgA,IgG,IL-17,TNF-α,ALB levels and 24 h urinary protein quantification in the study group were significantly lower than those in the control group,and APTT and PT were significantly longer than those in the control group(P<0.05).Conclusion Conventional drugs combined with low molecular weight heparin calcium in the treatment of HSPN in children has significant effect.It can shorten the disappearance time of clinical symptoms,improve immune function,coagulation function,renal function and alleviate inflammatory reaction.
