摘要
目的探讨海藻酸钠口腔用水凝胶辅助牙周机械治疗在Ⅱ/Ⅲ期牙周炎的疗效。方法将33例Ⅱ/Ⅲ期牙周炎患者随机分为三组:海藻酸钠口腔用水凝胶组;盐酸米诺环素软膏组以及单纯牙周机械治疗组,每组11例。所有患者均分次进行超声龈下刮治、根面平整以及抛光,其中海藻酸钠口腔用水凝胶组和盐酸米诺环素软膏组分别在牙周机械治疗后牙周袋内使用海藻酸钠口腔用水凝胶或盐酸米诺环素软膏,单纯牙周机械治疗组仅进行牙周机械治疗。三组患者在牙周基础治疗前和治疗后3个月,进行牙周检查。同时使用酶联免疫吸附法检测治疗前、治疗后1周、治疗后3个月龈沟液炎性因子TNF-α、IL-1β浓度。结果三组患者在基线时各项牙周临床指标以及龈沟液炎性因子TNF-α、IL-1β浓度均无显著性差异(P>0.05),三组患者在治疗前后各项指标自身对比均有明显缓解(P<0.05),其中海藻酸钠口腔用水凝胶组各项指标均比单纯牙周机械治疗组显著缓解(P<0.05),但海藻酸钠口腔用水凝胶组与盐酸米诺环素软膏组两者之间无显著性差异(P>0.05)。结论海藻酸钠口腔用水凝胶与盐酸米诺环素软膏均是治疗慢性牙周炎有效的药物,两者的疗效无明显统计学差异。
Objective To compare the curative effect of sodium alginate oral hydrogel as adjunctive treatment in the periodontitis grading of Ⅱ/Ⅲ.Methods Thirty-three patients with the periodontitis grading of Ⅱ/Ⅲ were randomly divided into three groups,periodontal traditional(ultrasonic scaling and manual root planning)treatment group,minocycline hydrochloride ointment group or sodium alginate oral hydrogel group.All subjects were treated with periodontal mechanical treatment.Minocycline hydrochloride ointment group or sodium alginate gel group was auxiliary periodontal topical medication.Clinical attachment loss(CAL),bleeding index(BI)and probing depth(PD)were recorded at baseline and 3 months after non-surgical periodontal treatment hy the same examiner.The gingival sulcus fluid inflammatory factor concentration of TNF-α,IL-1 β was detected hy enzyme-linked immunosorbent method before and after treatment.Results Before treatment,the mean of each indices was not different among the three groups(P>0.05).But After treatment,among the three groups,the indices were significantly lower than those before treatment(P<0.05).The indicators of sodium alginate oral hydrogel were significantly improved compared with the periodontal traditional treatment group(P<0.05),hut there was no significant difference between the minocycline hydrochloride ointment and sodium alginate oral hydrogel(P>0.05).Conclusions Sodium alginate oral hydrogel and minocycline hydrochloride ointment are effective drugs in the treatment of the periodontitis grading of Ⅱ/Ⅲ.
