摘要
目的研究PDCA循环法的质量持续改进在病区医护人员手卫生管理中的应用。方法选取2018年6月—2020年5月在我院工作的98名医护人员作为研究对象,分别来自外科(30名)、内科(30名)、针康科(20名)及重症医学科(18名)。各科室医护人员均应用PDCA循环法管理。比较各科室医护人员实施前后正确洗手情况、洗手依从性及手细菌检测情况。结果实施后外科、内科、针康科及重症医学科医护人员的正确洗手率[(83.95%vs.46.98%)、(83.12%vs.50.30%)、(81.90%vs.47.29%)、(81.07%vs.46.67%)],洗手依从性[(74.30%vs.33.80%)、(74.21%vs.31.71%)、(82.90%vs.31.43%)、(81.03%vs.35.24%)]及手细菌合格率[(83.33%vs.33.33%)、(86.67%vs.30.00%)、(90.00%vs.40.00%)、(94.44%vs.38.89%)]均高于实施前(P<0.05)。结论PDCA循环法的质量持续改进可促使医护人员做到正确洗手,并提高手卫生依从性,保障手细菌合格率,进而提高手卫生管理质量,有助于降低医院感染的发生风险,可行性高。
Objective To study the application of PDCA cycle method in hand hygiene management of medical staff in ward.Methods A total of 98 medical and nursing staff from June 2018 to May 2020 in our hospital were selected as the research objects,including surgery(30 cases)internal medicine(30 cases),acupuncture and health care(20 cases)and critical care medicine(18 cases).PDCA circulation method was used to manage the medical staff in each department.The situation of correct hand washing,hand washing compliance and hand bacteria detection of medical staff in different departments before and after implementation were compared.Results After the implementation,the correct hand washing rate[(83.95%vs.46.98%),(83.12%vs.50.30%),(81.90%vs.47.29%),(81.07%vs.46.67%)],hand washing compliance[(74.30%vs.33.80%),(74.21%vs.31.71%),(82.90%vs.31.43%),(81.03%vs.35.24%)]and qualified rate of hand bacteria[(83.33%vs.33.33%),(86.67%vs.30.00%),(90.00%vs.40.00%),(94.44%vs.38.89%)]in surgery,internal medicine,acupuncture and health care and critical care medicine was higher than before(P<0.05).Conclusion The continuous quality improvement of PDCA cycle method can promote the medical staff to wash hands correctly,improve the hygiene compliance of experts,ensure the qualified rate of hand bacteria,and improve the quality of health management of experts,which is helpful to reduce the risk of hospital infection and has high feasibility.
