CT与MRI诊断胸腹部孤立性纤维性肿瘤的影像特点及价值分析

Imaging characteristics and value analysis of CT and MRI in the diagnosis of solitary fibrous tumors in the chest and abdomen

摘要 目的分析胸腹部孤立性纤维性肿瘤(SFTs)疾病特点,总结CT与磁共振成像(MRI)的诊断价值。方法85例胸腹部SFTs疾病患者,均予以CT与MRI诊断,以病理诊断结果作为金标准,统计比较MRI、CT诊断胸腹部SFTs疾病的效能以及影像学特征。结果MRI诊断胸腹部SFTs患者肺内、胸膜、腹膜、后腹膜肿瘤数量分别为43、31、26、8枚,良性肿瘤95枚,恶性肿瘤13枚;CT诊断胸腹部SFTs患者肺内、胸膜、腹膜、后腹膜肿瘤数量分别为33、30、18、8枚,良性肿瘤87枚、恶性肿瘤2枚。MRI诊断胸腹部SFTs患者各病位肿瘤效能优于CT,且对良性肿瘤检出率100.00%、恶性肿瘤检出率86.67%均高于CT的91.58%、13.33%,差异均具有统计学意义(P<0.05)。胸腹部SFTs患者影像学特征方面,CT检查包膜相对完整,有肺内肿瘤块影,边缘光滑、密度均匀、无毛刺,强化影像后肿瘤轻度不均匀,胸膜肿瘤呈椭圆/卵圆形,腹膜病位边缘轻度强化、密度均匀、包膜完整。MRI检查肺内肿瘤信号均匀、包膜完整且光滑,增强后延时性强化。结论CT与MRI均可用于诊断胸腹部SFTs疾病,影像特点明显,MRI优势突出,为了提升诊断效能,建议联合诊断。 Objective To analyze the characteristics of solitary fibrous tumors(SFTs)in the chest and abdomen,and summarize the diagnostic value of CT and magnetic resonance imaging(MRI).Methods All 85 patients with SFTs disease in the chest and abdomen were diagnosed by CT and MRI,and the pathological findings were used as the gold standard to statistically compare the efficacy of MRI and CT in diagnosing SFTs disease in the chest and abdomen and the imaging characteristics.Results MRI diagnosed the number of lung,pleura,peritoneum,and retroperitoneum tumors in patients with SFTs in chest and abdomen were 43,31,26,and 8 tumors,respectively,with 95 benign tumors and 13 malignant tumors;CT diagnosed the number of lung,pleura,peritoneum,and retroperitoneum tumors in patients with SFTs in chest and abdomen were 33,30,18,and 8 tumors,respectively,with 87 benign tumors and 2 malignant tumors.The efficacy of MRI in diagnosing tumors at various sites in patients with SFTs in the chest and abdomen was better than that of CT;the detection rate of MRI for benign tumors was 100.00%and malignant tumors was 86.67%,which were higher than 91.58%and 13.33%of CT;all the differences were statistically significant(P<0.05).Regarding the imaging features of patients with SFTs in the chest and abdomen,the envelope was relatively intact on CT examination,there was a tumor mass shadow in the lung with smooth margins,uniform density,and no burr,and the tumor was mildly uneven on enhanced images,the pleural tumor was elliptical/oval,and the peritoneal site had mildly enhanced margins,uniform density,and intact envelope.MRI examination of intrapulmonary tumor with uniform signal,intact and smooth envelope,and delayed enhancement after enhancement.Conclusion Both CT and MRI can be used for the diagnosis of SFTs in the chest and abdomen,with obvious imaging characteristics and outstanding advantages of MRI.Combined diagnosis is recommended in order to enhance the diagnostic efficacy.
